[Abstract] Objective To verify the important value of meridian identification in clinical practice by taking acupuncture treatment of radial styloid tenosynovitis as an example Method 67 patients with radial styloid tenosynovitis were selected for simple acupuncture treatment, randomly divided into 33 cases in the control group and 34 cases in the treatment group. The total effective rate of the treatment group and the control group were 100% and 81.82% respectively, and the cure rate was 52.94% and 9.09% respectively, and the difference was statistically significant when comparing the clinical efficacy between the groups (P “0.05) Conclusion The use of meridian identification can accurately determine the meridians and internal organs where the disease is located in clinical practice, so that the acupuncture points can be selected in a more targeted manner with significant efficacy.

Meridian identification is based on the physiopathology of the meridians and their associated internal organs, and identifies the diseases manifested in the meridians and their associated internal organs, so as to clarify the nature of their symptoms.

In order to verify the clinical value of meridian identification, a common clinical disease, radial eminence tenosynovitis, was selected for simple acupuncture treatment as a clinical application study of meridian identification.
The reason for choosing radial eminence tenosynovitis as the treatment type to study meridian identification is that it is a common clinical disease with universality and representativeness, and because the diagnosis and treatment are simple and can be treated with acupuncture alone.
The third reason is that only the superficial and internal channels of the radial eminence of the radial lung meridian and the Yangming large intestine meridian circulate, so the meridian circulation is simple and the selection of acupuncture points for meridian identification is relatively simple.
Both can fully respond to the purpose of the experiment, but also has a strong experimental replicability, scientific stronger.

1. Case selection
1.1 Diagnostic criteria The diagnosis of radial styloid tenosynovitis was made with reference to the diagnostic criteria of Note 2 of Chinese Medicine Injury Science and Note 3 of Huang Jiaji Surgical Science.
1.2 Inclusion criteria 1 with radial pain at the wrist for more than one month as the main symptom 2 Age 18-65 years old 3 with stable vital signs, clear consciousness, ability to express and perform, and able to cooperate with treatment and clinical observation. 4 with stable conditions of other intercurrent diseases.
1.3 Exclusion criteria 1 with serious physical illness 2 with mental illness 3 with malignant tumor 4 with sudden onset of other acute illnesses during treatment 5 with a history of alcohol consumption and alcohol consumption greater than 140G/week 6 who were not treated according to the regulations and whose data were incomplete to judge the efficacy.

Clinical data 67 patients were observed in the Department of Traditional Chinese Medicine, Tongji Hospital, Tongji University School of Medicine, Shanghai, China, from March 2010 to June 2012, and were divided into treatment and control groups by the random number table method. 34 patients were in the treatment group, including 7 males and 27 females, with an average age of (53.67+-13.37) and an average disease duration of (3.7+-0.7) months, while the control group There were 33 cases, including 6 males and 27 females, with a mean age of (52.76+-12.33) years and a mean duration of illness of (3.3+-0.6) months. There was no statistically significant difference between the two groups in terms of age, sex, and duration of disease at baseline (P>0.05), which was comparable.
3, Treatment method Acupuncture treatment alone (without any internal and external drugs and any other adjuvant treatment) once every three days, fifteen times as a course of treatment

1.3.2 Treatment group: Shaoshang, Shang Yang, trigeminal acupuncture and bleeding, acupuncture (flat tonic and flat drainage), Taiyuan, Yili. After obtaining Qi, the needles are kept for about 20 minutes.
1.4 . Evaluation of efficacy Refer to “Diagnostic and efficacy criteria for Chinese medicine” Note 6
Cured: disappearance of swelling and pressure pain on the radial side of the wrist, restoration of function, negative clenched fist ulnar deviation test.
Improving: wrist pain is reduced, slight pain when moving, suspicious positive fist and ulnar deviation test.
Ineffective: no improvement of symptoms.
1.5 Statistical methods The test data were statistically analyzed using SPSS11.5 software package. The t-test was used for measurement data, and Radit analysis was used for rank data.

According to the theory of meridians, the channels and pathways that communicate between internal and external surfaces are the meridian system, and each meridian has specific internal organs and specific circulation routes from the surface to the inside.
If a specific circulation part of the body is found to be diseased, then theoretically we can accurately determine which meridian is diseased on the outside and which organ is diseased on the inside.
This is where the significance of meridian identification lies.

Therefore, the pathogenesis of tendinitis of the radial styloid process is mainly based on the meridian of the hand Taiyin meridian, which is mutually related to the yin and yang of the internal organs.
The main reason for the pathogenesis of radial styloid tenosynovitis is the hand-taiming meridian, and the internal organs are the lung and large intestine, which are also mutually related to the yin and yang.

The connotative meaning of the selection of points for meridian identification: Shaoshang is a well point of the hand Taiyin lung meridian, Shang Yang is a well point of the hand Yangming large intestine meridian, puncture bleeding to take the meaning of heat release, Tai Yuan is the original point of the hand Taiyin lung meridian, and
This is not only a method to match the original meridian points of Yin and Yang, but also because the disease is fundamentally a disease of deficiency, so more attention should be paid to regulating the internal organs and meridians, not to drain heat too much, and thirdly, to unblock the local foci of the disease.
From the actual clinical efficacy, the treatment group using acupuncture points with meridian identification has an obvious and important advantage over the control group with acupuncture points near the part, both in terms of total efficiency and cure rate.
Therefore, with the mastery of meridian identification, it is theoretically possible to determine the meridians and internal organs where the disease is located as accurately as the drums should be, so that acupuncture points can be selected in a more targeted manner to achieve more significant curative effects.

Of course, due to the complexity of the disease, in order to understand the truth of the disease as completely as possible, the four diagnostic methods of examination and diagnosis should not be neglected, and the meridian identification should be applied in combination with all the identification methods of Chinese medicine, reflecting each other, in order to explore the truth of the disease.
In addition, as I said at the beginning of my article, I chose radial eminence tenosynovitis as the disease type for meridian identification because it is simple and easy to illustrate the principle of meridian identification and the replicability of the experiment, in fact, because meridian theory is an important theoretical basis of TCM, so meridian identification can be applied to all TCM departments, such as dermatology, orthopedics, surgery, internal medicine, gynecology, pediatrics, etc. Meridian identification can be used in all TCM departments. This is exactly what Yu Jiayan said in “The Law of Medicine”: “If you do not know the internal organs and meridians, you will be wrong if you open your mouth and do something”.
