In clinical practice, we often see some patients with compression fractures of the thoracolumbar segment with compression degree around 1/4 to 1/3 without neurological symptoms, who are often afraid of surgery, or for economic reasons, their first choice will not be surgical treatment, but are willing to receive conservative treatment without being able to carry out medical advice well. After about a month of lying flat, the pain is relieved and they start to get out of bed, leading to an increased rate of complications such as low back pain, spinal stenosis, and retrobulbar deformity in the later stages of conservative treatment. Recently, we have encountered several patients with related afflictions, many of whom came to the hospital later for review and found that old lumbar fractures had developed a posterior convexity deformity and intractable lumbar pain, either because they did not come to the hospital after the fracture and therefore did not lie down as required, or because they did not follow the doctor’s orders. For thoracolumbar fractures, bed rest is generally more than 8 weeks, and Sam Lee personally recommends that you stay in bed for 12 weeks with a hard board bed. You can lie flat with a soft pillow on your lower back, or you can lie prone. Lateral lying is also possible, but no twisting and bowing. During the conservative treatment period, do not get out of bed and sit in a semi-recumbent position. When turning over, the shoulders and hips should be kept in a straight line without twisting, that is, axis turning. After a week, the pain will be relieved and you can exercise the strength of the lumbar back extensors in the prone position. Or supine position with both feet and elbows support. These exercises, in addition to increasing the strength of the low back muscles, also allow the anterior longitudinal ligament in front of the vertebrae to stretch, thus restoring the height of the compressed vertebrae. In addition to low back muscle exercise, can also be carried out in the lower extremities of non-weight-bearing lifting exercise, foot bicycle-like exercise, no rated amount, to not tired appropriate. You can frequently massage the lower limbs and move the joints of lower limbs in bed to prevent deep vein thrombosis of lower limbs. Special emphasis: bed rest is not the concept of not working at home and lying in bed to rest, but can only take the position of lying in bed, urination and defecation can not get out of bed. What’s more, you can’t sit in bed. Half-lying reclining also can not. This is the concept of absolute bed rest.
