Common clinical manifestations Visual distortion, gradually aggravated, central dark spot, significant loss of visual acuity, the disease if not treated in time, there will be a continuous loss of visual acuity, visual distortion, etc. If the macular area bleeding for a long time or repeated bleeding, will seriously affect the visual function, the prognosis is worse, or even blindness. Funduscopic examination reveals hemorrhage in the macular area of the eye, accompanied by yellowish-white exudate spots. Ancillary examinations Ocular tomography (OCT) examination reveals lesions below the retina in the macula, which may lead to corresponding retinal bulging. On fundus vascular and choroidal angiography, punctate lesions are seen in the macula initially, with expanding leakage in later stages. The neovascularization from the choroid invades the Bruch’s membrane and can form a neovascular network in the deep retinal layer. Treatment: 1. Photodynamic therapy (PDT) of choroidal neovascularization Photo-sensitizers cause blockage of blood vessels by photochemical reaction (non-thermal effect) under the excitation of lower energy and specific wavelength of light, resulting in gradual atrophy of choroidal neovascularization. Overseas Verteporfin (Vetiporfin) for choroidal neovascularization has been approved by FDA for clinical use. At present, PDT is the only effective means to treat CNV in the macular region of highly myopic eyes in most large hospitals in China, which can effectively control the development of the disease and play a great role in stabilizing or even restoring vision. In China, PDT is used to treat CNV in the macular region of highly myopic eyes, and the progressively enlarging lesions gradually shrink and leakage gradually decreases after treatment, and most of these patients have stable or even improved visual acuity. 2, vitreous cavity injection of vascular endothelial growth factor antibody Avastin (Avastin) or Lucentis (razumab injection), some large hospitals can recently get a small amount of razumab injection. 3. Combined treatment of the above two methods is suitable for severe and persistent affected eyes.
