What are the steps in the surgical approach to removing lipomas

Lipomas can be categorized into surface lipomas and deep lipomas. The resection steps are generally preoperative anesthesia, preoperative preparation, surgical steps, postoperative care and other items. 1. Preoperative preparation: Prepare the skin of the operation area, clean the local skin of the operation site, and remove the hair if there is any. 2. Preoperative anesthesia: local anesthesia is usually used for removing surface lipoma, while subdural anesthesia is often considered for deep lipoma. 3. Surgical procedure (1) Surface lipoma: make an incision along the direction of skin lines, directly reaching the peritoneum. Then bluntly separate the tissues, strip out the lipoma and ligate the hemostatic suture, pay attention to avoid leaving dead space. (2) Deep lipoma: perform lumpectomy under continuous epidural anesthesia. Make a curved incision on the outer side of the local skin of the mass, open the deep skin fascia, fully separate the tissue gap, expose the lipoma and then peel it off, then excise it and then suture it layer by layer, and the excised tissues need to be sent to pathology for examination during the operation. 4. Postoperative care: some larger lipomas should be bandaged with pressure after resection to prevent hematoma. Secondly, patients who placed rubber strip drainage should be removed after 24~36 hours. Antibiotics can be routinely used to prevent infection. If there is a history of lipoma, it is recommended to go to the hospital for examination and listen to the doctor’s opinion to decide whether excision treatment is needed.
