Pediatric “snapping fingers” are also known as stenosing tenosynovitis or trigger finger. It is commonly thought that tenosynovitis is mostly seen in adults and is related to strain. In fact, it is not uncommon in children. In children with snapping fingers, the symptoms do not appear at birth, but usually occur at the age of 6 months to 2 years when the fingers are flexed and cannot be straightened, with the most common sites being the metacarpophalangeal joints of the thumb, second and third fingers. The “popping finger” is a common congenital deformity in children, due to the thickening or nodularity of the proximal tendon of the flexor tendon due to unreasonable friction or other reasons, and the narrowing of the tendon sheath fibrocartilage ring lesion, which causes a ringing sound when the finger is flexed and extended in mild cases; in severe cases, the joint is in a flexed position and cannot be actively straightened, causing pain or popping when it is passively extended. According to years of clinical observation and treatment experience, congenital tendon sheath stenosis cannot heal by itself and will easily affect the development of the hand and thumb flexor muscles in children, so treatment should be given as early as possible. In a small number of children, local closure or topical application of medication can be effective, but in most cases the above measures are not ideal. Massage is also effective, but the treatment usually takes more than a month and the healing rate is low. Surgical treatment is more thorough. Surgery is usually performed by making a transverse incision on the palmar side of the metacarpophalangeal joint and cutting the narrow tendon sheath longitudinally. The surgery is not complicated. The stitches are removed 2 weeks after surgery, and the basic recovery time is usually 1-2 months. Medical costs, according to the medical fee standard of Zhejiang Province, the total medical costs before and after the surgery for stenosing tenosynovitis in a general tertiary hospital is about 2000 RMB. Personally, I recommend small acupuncture treatment. The treatment time of small needle knife is 5-10 minutes. After the treatment, the popping sound disappears immediately when the finger is flexed and extended, and the flexion and extension are free. There is basically no pain, very little bleeding, and no scarring. The success rate is over 85%. Our hospital is a tertiary hospital, and the total cost is usually around 200-300 RMB.
