How are common rheumatic diseases treated in Chinese medicine?

       Rheumatoid arthritis
Rheumatoid arthritis is characterized by painful swollen joints and stiffness and deformity of the joints in the late stage, which is the category of “paralysis” in Chinese medicine. The disease is similar to the “joint disease” and “bone paralysis” in paralysis.
The pathogenesis of this disease has the following characteristics in addition to the general rules of paralysis: 1.
1, joint swelling, early joint swelling is mainly due to dampness, or the evil of cold and dampness, or the evil of dampness and heat; the course of the disease is long, the swelling is mostly due to dampness does not go, gathered into phlegm, and mixed with blood stasis, so the pathogenesis of the swelling is mostly related to phlegm and blood stasis.
  2. The long duration of the paralysis disease, the exhaustion of the whole, in addition to causing deficiency of qi and blood often involves the liver and kidney, resulting in liver and kidney deficiency, which is closely related to the liver main tendon, kidney main bone.
Because the early stage of the disease is dominated by the actual evil, treatment to eliminate the evil first; to the middle and late stage of the evil stay is injured, need to treat both the symptoms and the root cause, to eliminate the evil and help the balance.
1, damp-heat paralysis evidence: Sanmiao Wan combined with Xuanzhuo Tang plus reduction.
2, cold-damp paralysis: Wu Tou Tang combined with Fang Ji Huang Qi Tang plus or minus
3.Phlegm and stasis obstruction: Body pain and stasis removal soup with Xiaohuoluodan, plus or minus
4, deficiency of liver and kidney, paralysis of tendons and bones: tonifying the kidney and strengthening the governor to treat short orchid pupa
  Ankylosing spondylitis
Ankylosing spondylitis is mainly manifested by lumbosacral pain, morning stiffness and restricted lumbar movement, and belongs to the category of “paralysis” in Chinese medicine. It is similar to “lumbago”, “kidney paralysis”, “spinal strength” and other diseases.
The etiology of this disease has the following characteristics, in addition to the general rules of paralysis.
1, in the etiology of the disease and the congenital endowment has a close relationship, such as congenital endowment deficiency, kidney qi deficiency is prone to this disease.
2, the clinical manifestations of this disease are mainly lumbosacral pain, lumbago, and the disease of lumbago often has different degrees of kidney deficiency, as the Jing Yue Quan Shu says: “deficiency evidence of lumbago, eight out of ten.” Therefore, this disease is based on kidney deficiency, and the real evil such as cold and damp, damp and heat, rain and snow is the standard.
3, the disease is manifested by lumbar spine ankylosis, lumbosacral or lumbar back pain, morning stiffness and other symptoms, the pathogenesis is always related to blood stasis. The treatment of ankylosing spondylitis, while eliminating evil, should take into account the kidney deficiency according to the urgency of the disease, and the method of activating the blood to eliminate blood stasis is often used throughout the treatment process.
The following are common in clinical practice.
1, cold and damp paralysis evidence: Wu Tou Tang combined with kidney and Tang plus reduction
  2. Damp-heat blockage of ligaments: Sanmiao Wan with reduction
  3. Blood stasis obstructing the ligaments: Body pain and stasis removal soup with addition and reduction
  4. Deficiency of kidney energy: Qing’e Wan, plus or minus
According to the etiology and clinical manifestations of ankylosing spondylitis, our department uses San Piao Tang as the basis, and after adding and subtracting the corresponding drugs to form the formula of warming the kidney and opening the ligaments, which can alleviate the pain symptoms of patients and reduce the amount of western medicine, and has fewer side effects.
  Dryness syndrome
Dry syndrome is an autoimmune disease with exocrine gland lesions as the main cause. Dry mouth and dry eyes are the most common clinical symptoms, and may be accompanied by joint pain, and some patients may involve the lung, liver, kidney and other organs. Dryness syndrome belongs to the category of “dryness evidence” in Chinese medicine. This disease is called “paralysis of dryness” in “Treatments of Paralysis”.
The characteristics of its etiology and pathogenesis are as follows
  1. The cause of the disease may be due to external dryness, or due to the loss of nourishment after a long illness, depletion of fluids and insufficient essence and blood, resulting in internal dryness. “Dryness is dryness”, so clinical symptoms such as dry mouth, dry eyes or dry skin are seen.
2.Insufficient fluid for a long period of time, each leads to blood stasis, and interspersed with blood stasis manifestations, or involves the internal organs.
  Clinically common.
1.Yin deficiency of spleen and stomach: Yigong Tang with Yu Nian Decoction plus reduction
2.Yin deficiency of liver and kidney: Qiju Dihuang Wan
Clinically, patients with dry syndrome are more prone to malignant lymphoma than healthy people. The self-developed formula of detoxification, enrichment, and promotion of fluid in our department can significantly improve the symptoms of dry mouth and dry eyes of patients and reduce immunoglobulin. At present, our department has carried out extensive clinical and basic research on this formula.
  Systemic lupus erythematosus
SLE has various clinical manifestations, such as joint pain, skin erythema, fever, edema, palpitations, etc. Therefore, different patients with different disease processes can be associated with “paralysis”, “butterfly sore”, “yin-yang toxicity”, “yin-yang toxicity”, and “yin-yang toxicity” in Chinese medicine. Therefore, the different disease processes in different patients can be similar to “paralysis”, “butterfly sore”, “yin-yang toxicity”, “edema” and “deficiency labor” in Chinese medicine.
Because of the diversity of clinical manifestations of SLE, such as generalized fever, joint pain, skin erythema, and involvement of multiple organs, its etiology is particularly complex.
  Its characteristics.
  1. The disease mostly sees fever, or high fever that does not subside, which is mostly related to the feeling of wind, heat, fire and dryness among the six mirths, etc. Evil heat invades and fills the inside and outside, so fever is seen.
  2. The evil heat injures the blood vessels, forming the symptoms of blood heat and blood stasis, which are seen as erythema on the face and hands, mouth ulcers and pain in the limbs.
  3. The disease often involves the five viscera, resulting in damage to the five organs, palpitations, edema, asthenia and epilepsy, of which weakness of spleen qi and deficiency of kidney yin are most common.
       Clinically seen as.
1, evidence of incandescence of heat and toxicity: clearing plague and defeating toxin drink plus reduction
2, paralysis by heat and stasis: Si Miao Yong An Tang combined with Body Pain and Stasis Removal Tang plus or minus
3, Yin deficiency and internal heat: Zhi Bai Di Huang Wan, plus or minus
4.Spleen and kidney yang deficiency: Jisheng Kidney Qi Pill combined with Zhenwu Tang, plus or minus
  Osteoarthritis
Osteoarthritis mainly involves weight-bearing joints such as the cervical spine, lumbar spine, knee joints, etc. Osteoarthritis of the finger joints is mostly related to heredity. If the cervical spine is involved, it is called cervical spondylosis. Therefore, the various manifestations of osteoarthritis are similar to those of “paralysis”, “lumbago”, and “numbness” in Chinese medicine.
The onset of osteoarthritis is mostly seen in middle-aged and elderly people, and its incidence increases with age. The clinical manifestations are mainly painful and the pain is fixed.
Therefore, the etiology and pathogenesis of this disease have the following characteristics.
  1. As the elderly age, their liver and kidneys become increasingly deficient, and deficiency of the liver and kidneys is the root of the disease.
  2. Heavy pain, fixed pain, or even stabbing pain, are the manifestations of blood stasis. If the joints are swollen, or if there is joint effusion, it is a damp-heat paralysis, and the treatment should be based on clearing heat and relieving dampness.
  Clinically, we can see that
  1. Blood stasis and obstruction: Peach kernel and four ingredients soup with reduction
2, deficiency of liver and kidney: Qing’e Pill combined with Right Return Pill
  3. Damp-heat paralysis: Si Miao Wan with addition and reduction
Patients with SLE take hormones for a long time, causing various kinds of side effects. Chinese medicine can reduce the side effects of hormones and improve the clinical efficacy by combining with western medicine.
  Leukoarthrosis
Leukoaraiosis is a disease of the rheumatic immune system, specifically autoimmune connective tissue disease.
Leukoaraiosis (leukoaraiosis syndrome) is a chronic progressive recurrent multi-system inflammatory disease of unknown etiology, with fine vasculitis as the pathological basis and ulcers of the mouth, eyes, external genitalia and skin as the main manifestations.
The descriptions of similar symptoms in Chinese medical writings were first published in Zhang Zhongjing’s “Treatise on Typhoid and Miscellaneous Diseases” in the Later Han Dynasty and later collected in the “Essentials of Jin Kui”. The disease is called “fox confusion disease” because of the variety and unpredictability of the symptoms, especially like “fox confusion”, and it is said that “the throat is confused and the yin is fox”. Chinese medicine believes that this disease is caused by external dampness and heat, and internal deficiency fire.
At present, its etiology and pathogenesis are not yet clear, its clinical features.
  1. Almost 100% of oral ulcers occur. The ulcers are distributed on the tongue, gums, inner side of upper and lower lips, oral mucosa or throat, and are prone to recurrent attacks.
  2. The incidence of ocular symptoms is 70%, with a higher and heavier incidence in men, such as keratitis, corneal ulcer, sclerositis, chorioretinitis, and typically often accompanied by iridocyclitis or even blindness.
3, external genital ulcers, the incidence of 66.8%, genital, skin, mucous membrane can occur ulcers, large and deep, pain, slow healing.
  4. Skin damage occurs in 76% of cases. Impetigo, folliculitis, boils and erythema can occur.
  5. Arthralgia and heart, lung, kidney and intestinal tract are involved. Increased blood sedimentation and mildly elevated blood cells.
Clinical treatment can be based on dialectic evidence, such as licorice diarrhea heart soup.
