Poor bone marrow function, unable to tolerate postoperative adjuvant chemotherapy, and prolonging the survival of patients with middle and advanced gastric cancer after surgery Case 2: The patient was male, 68 years old, and consulted a hospital in Beijing in July 2001 because of unexplained discomfort in the gastric epigastric region and dizziness, and gastroscopy showed that the gastric sinus was occupying a position, and biopsy showed that gastric cancer was present. “Gastric cancer was resected on July 30th at the Cancer Hospital of the Academy of Medical Sciences, and the postoperative pathology showed “low-differentiated adenocarcinoma of the gastric sinus, invading the muscular layer and reaching the extra-plasma membrane fat, with lymph node metastasis of the side of the greater curvature of the stomach in 6/19”, Stage IIIa. One month after the operation, she started 3 cycles of chemotherapy with 5-Fu 13.5g, E-ADM 270mg, PDD 450mg. She suffered from myelosuppression of the second degree, with the lowest WBC of 2,300/mm3. She injected granulocyte colony-stimulating factor for more than 10 days, and the white blood cells (WBC) rebounded, and after stopping the injection of hematopoietic injection, the WBC had been in the range of 2-3.0×109/L. The tongue was pale red, the moss was yellow and greasy, and the pulse was weak. Laboratory examination: WBC 2.7×109/L. Tumor markers CEA and Ca199 were normal. Diagnosis: deficiency of heart and spleen. Second diagnosis: the patient’s fatigue was reduced, appetite increased, abdomen was not distended, occasional fatigue, appetite was acceptable, sleep was poor, and bowel movements were adjusted. The tongue was dark, the moss was thin and yellow, and the pulse was fine. WBC 3.7×109/L. The patient refused chemotherapy and hoped to continue with traditional Chinese medicine. Diagnosis: Qi deficiency and blood stasis. Formula: Six Gentlemen Soup + Angelica Sinensis Blood Replenishing Soup Plus Reduction, Crown Prince Ginseng 15 g, Fried Atractylodes Macrocephala 12 g, Poria 15 g, Astragalus Membranaceus 30 g, Radix Angelicae Sinensis 6 g, Acacia Pi 15 g, Corydalis Macrocephala 18 g, Pericarpium Citri Reticulatae 10 g, Semen Armeniaca Purpurea 9 g, Fried Citrus Aurantium Lobatae 9 g, Rhizoma Codonopsis Pilosulae 15 g, Vitis Vinifera Lobatae 18 g, Scutellariae Barbatae 10 g, Fructus Cynomolae 18 g, Semen Armeniacae 30 g, Semen Armeniaca Sinensis 15 g, Fructus Bupleurum 30 g, Radix glycyrrhiza Uralensis 6 g. In November 2001, the patient was hospitalized in the ward of our department and started to receive intravenous drip of Chinese medicine injection to help her fight against cancer. Since then, while the patient was taking oral identification Chinese medicine, he came to our hospital for hospitalization every year for 2~3 months for 3 years after the operation to take intravenous drip of Eleutherococcus lactis injection and thymus peptide. After 3 years of postoperative Chinese medicine treatment, the blood image had been maintained at the normal level without any chemotherapy, the tumor markers were normal in the reexamination, and no signs of metastasis and recurrence were found in the gastroscopy and imaging examination. In the 4th year after the operation, the patient came to the hospital every 3~4 months for intravenous drip of Huachunin injection + identification of traditional Chinese medicine. 5 years after the operation, the patient was no longer hospitalized, but he had been taking the identification of traditional Chinese medicine and our hospital preparation Xihuang detoxification capsule orally in the outpatient clinic, and he was also taking the Huachunin tablets in spring and autumn, and the quantity of the identification of the soup was reduced to a single dose in 3~4 days after the operation for 7 years, and the patient was comprehensively examined every year, and he has been surviving for more than 10 years, and there were no signs of relapse and metastasis in the comprehensive examination, and was clinically cured. There is no sign of recurrence and metastasis in the comprehensive review, and the patient is clinically cured. Comment: This patient has IIIa gastric cancer, with 3 cycles of postoperative chemotherapy, but failed to complete the cycle of chemotherapy systematically due to poor bone marrow function. The patient’s poor bone marrow function belonged to innate endowment insufficiency, coupled with postoperative qi and blood deficiency as well as the patient’s strong demand for simple traditional Chinese medicine treatment, we chose the individualized treatment plan of traditional Chinese medicine based on the identification of traditional Chinese medicine, and at the early stage of the treatment the patient belonged to the post-chemotherapy period, and used the method of benefiting the qi and nourishing the blood as the main treatment, the patient’s symptoms of fatigue and poor appetite were improved, and qi and blood were regulated, and the body quality was restored, and the blood picture was restored to normal. In addition, considering that the patient’s evidence belongs to qi deficiency and blood stasis, on this basis, Tiger Cane, Garcinia Cambogia Root, White Flower Snake Tongue Herb were added to invigorate blood and detoxify the blood to fight against cancer, and at the same time, Eleutherococcus Emulsifiable Lactate was applied to strengthen the power of invigorating blood and fighting cancer. Elemene Emulsion is a new national class II anticancer drug extracted from China’s herbal medicine Wen Curcuma, with β-elemene as the main ingredient. The New Compilation of Materia Medica (本草新编) says: “Curcuma longa is bitter, pungent, warm and non-toxic. It enters the liver and spleen meridians.” It is bitter, pungent, dispersing and warm, which enters the blood division and the qi division, and can break the blood and disperse the blood stasis, eliminate the accumulation of Y, and move the qi to relieve pain. Previous clinical and basic experiments have confirmed that this injection has the effect of activating blood circulation, relieving pain and anti-cancer, and may have better efficacy in treating gastric cancer, especially for patients with blood stasis. Therefore, the patient has been adhered to the treatment with this injection and discriminating traditional Chinese medicine for 3 years after operation and chemotherapy. In the 4th year after surgery, the patient had no obvious clinical symptoms, which was “no evidence to identify” according to Chinese medicine theory, but considering the patient’s history of gastric cancer, there were still relevant factors in the body that caused gastric cancer cells, which belonged to “residual toxins not yet exhausted”, and the treatment should adopt the therapeutic principle of “supporting correctness and eliminating toxins”. The treatment should be based on the principle of supporting and detoxifying. We used Huachunin injection, which is extracted from the skin of Chinese toad, and experimental research has proved that it can improve the immune ability of the body, and in vitro test has a killing effect on many digestive tract tumor strains. Therefore, we chose this injection to detoxify and fight against cancer as one of the main consolidation treatments for 4~5 years after the operation. The patient successfully passed 5 years after the operation, and was cured in principle without any need for further treatment, but considering that the patient failed to complete the chemotherapy systematically, we suggested that the patient should take oral TCM in outpatient clinic intermittently, and add detoxifying and anticancer traditional Chinese medicines according to the different seasons, and he has survived until now. This patient has achieved long-term survival through the use of an individualized treatment plan based on her constitution, consisting of a TCM soup and an anti-cancer TCM injection. Although this case is a case report, it can be found from the treatment process that postoperative patients with intermediate and advanced gastric cancer may still achieve long-term survival by adhering to individualized TCM treatment.
