What is cervicogenic heart disease?

Clinically, we often see patients who have palpitations, chest pain, precordial discomfort, suffocation, shortness of breath and other symptoms of heart disease, and then referred to orthopedics and diagnosed as “cervicogenic heart disease”, but after treatment of cervical spondylosis, the symptoms are significantly improved or relieved. What exactly is cervicogenic heart disease? Cervical spondylosis is a common disease and high incidence among middle and old people, caused by cervical disc degeneration and cervical spine osteophytes, with neck and shoulder pain, radiating to the head and occipital region or upper limbs, resulting in headache and dizziness, numbness and pain of upper limbs, and in severe cases, spasm of both lower limbs, difficulty in walking, resulting in tetraplegia as the main manifestation of the syndrome. Cao Yafei, Department of Orthopedic Injuries, Shenzhen Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine, Zhengzhou City Hospital of Traditional Chinese Medicine, Department of Tui-Na, Kang Xiangyong As the research on cervical spondylosis progresses, it is found that in addition to the above-mentioned common symptoms, cervical spondylosis can often involve the cardiovascular system and symptoms of heart disease, such as pain in the precordial region, arrhythmia, chest discomfort, palpitations, shortness of breath, and so on. These cardiovascular diseases caused by cervical spondylosis are collectively referred to as “cervicogenic heart disease” or “cervical heart syndrome”. The pain in the precordial region is called “cervical angina” and the arrhythmia is called “cervical arrhythmia”. Cervicogenic heart disease is mainly caused by sympathetic nerves, which emanate from the cervical and thoracic spine and are then distributed to the heart to affect the heart’s activity. When the cervical spine is diseased, it will compress the sympathetic nerves, which will affect the nerves that innervate the heart and then cause heart disease, so it will show symptoms similar to coronary heart disease, angina, and some patients will have tachycardia, bradycardia and other heart disease symptoms. Although “cervicogenic heart disease” is often misdiagnosed as coronary heart disease. However, angina pectoris in cervicogenic heart disease is different from angina pectoris in coronary heart disease. It has nothing to do with exertional load and emotional excitement, and cannot be relieved by nitroglycerin drugs and calcium antagonists; however, the increased burden on the cervical spine is often the cause of this type of angina, such as high pillow position, maintaining an excessively head-up or head-down posture for a long period of time, turning the head and neck to one side for a long period of time, cold, moisture, sprains and strain on the spine. It is worth noting that before diagnosing cervicogenic heart disease, heart disease should first be ruled out through electrocardiogram, coronary angiography and other examination methods. The root cause of “cervicogenic heart disease” is cervical spondylosis, so the main treatment for cervical spondylosis. For example, a pillow of suitable height to avoid excessive strain on the cervical spine; pay attention to the warmth of the neck, prevent the neck and back from cold, appropriate neck gymnastics to move the neck; local physiotherapy, hot compresses, cervical traction, massage, Chinese medicine and other treatments can alleviate or reduce the symptoms of “cervicogenic heart disease”.
