What are the reasons why doctors are facing a “shortage of successors”?

The “white angel” is the praise of medical workers, but in recent years, affected by a variety of factors, this sacred profession is often misunderstood, and even make the people feel dissatisfied. Over the years, “Life Times” reporter in the process of dealing with many doctors learned that their work is not easy, but also has a variety of hardships. Some medical professionals mock themselves by saying that they are “shiny on the outside but haggard on the inside”; some even lament that “the most difficult profession in the world is to be a doctor in China”. Recently, the newspaper, together with Sohu.com Health Channel and Dingxiangyuan website, conducted a network survey and questionnaires among medical staff in 19 hospitals in 8 cities across China. We hope our report can help people better understand the real life situation of doctors and listen to their voices. In this issue, we started with a survey on “How Doctors are Judged by the People”, in which 2,067 netizens participated. The results show that being a doctor in China is not recognized by most people, and many people believe that they are less respected than in the past, have “low income” and “no time to take care of their families”, and that the team of doctors may even have “few successors “The survey shows that doctors are not as respected as they used to be. Doctors are not as respected as they used to be. The survey covered many aspects of doctors’ respectability in the eyes of the public, their income, and their family happiness index. When asked “how do you evaluate the profession of doctors”, 20.80% of people chose “white angel, very honorable”; 43.61% of people chose “hard work, no fun “. Obviously, people do not have a unified opinion on the evaluation of the profession of doctors, and the recognition is low. One netizen left a message after the survey: “I also think it’s not easy for doctors to see people non-stop. But when it’s my turn to see a doctor, I can’t help but feel dissatisfied with the doctor’s almost cold face, listening to impatient ‘orders’, and the expensive drugs in my hands when I leave the hospital. I understand their situation, but I cannot understand what they do. I think that the medical ethics of doctors are now much worse than before.” Earlier, a survey conducted by CCTV’s “Time and Space Survey” column showed that 94% of people believe that the reputation of doctors has declined compared to 10 years ago, and 41% believe that the main reason for this is that some doctors are overly pursuing economic interests, large prescriptions, excessive examinations, red packets and drug rebates. In this regard, Xia Xueluan, a professor of sociology at Peking University, told Life Time that doctors are a very sacred profession, respected and envied, which was more prominent and obvious in the past. “In fact, now, doctors are still our ‘white angels’, they bear the burden of saving lives and helping the injured. Only by the shortage of medical resources, doctor-patient conflicts and other factors, some medical workers and the people’s mentality has changed, and the public status of this profession is also somewhat slippery.” How do the people see doctors’ income, marriage So, what is the living situation of doctors in the eyes of the public? First, the income situation. When asked, “Do you think the income of doctors matches the effort?”, 69.55% of respondents think “low”, only 18.05% of respondents chose “black-hearted money” Only 12.40% of people chose “proportional”. In other words, although many people are dissatisfied with the work of doctors, but most of them also realize that the income of doctors is not commensurate with their hard work. “This result is somewhat surprising to me.” Hong Tao, a professor of cardiology at Peking University First Hospital, said, “It shows that the people are starting to think from the doctor’s point of view. And before that, some of the media and the people were much worse for us.” Xiao Chen, who works in the IT industry, told reporters that his high school classmate works as an attending physician in a tertiary care hospital in Beijing. “The monthly salary is just over 1,000 yuan, but the pressure is particularly high. We have several class reunions, he is busy because of work did not participate. Look at him so tired, but because of the love of the industry and insist not to jump ship, we are quite admired.” The financial situation of medical staff in rural health centers is even more difficult. “We contacted a rural doctor in the suburbs of Beijing.” Zhang Wei, an assistant professor in the Department of Health Economics and Management at Peking University’s Guanghua School of Management, recalled, “He was paid only 600 yuan a month and worked very hard to see others, suffering from anemia himself but not having enough time or money to get medical treatment.” A Sohu user has a different view on this, she said: “Although the legitimate income of doctors is low, but ‘gray income’ more ah. Both red envelopes, and kickbacks, the money does not come from the people’s pockets? Doctors can not be because they earn less, to earn the people’s money ah.” A pharmaceutical company, and doctors have dealt with years of pharmaceutical representatives are somewhat secretive to tell reporters: “Young doctors earn less, but wait to become a senior doctor, the opportunity to make money will be more.” Second, family and married life. When asked “are you willing to marry a doctor”, 65.71% of respondents said “no”. Among them, 82.5% said that the main reason for “unwilling” is “no time to take care of the family”; 66.9% thought that doctors are “always scolded by patients”; 33.3% chose “low income”. 33.3% chose “low income”. “Taking care of your family? I don’t really expect that. It’s good if he can come home for two dinners a week.” A family member of a doctor who did not want to be named said with a slight complaint that her husband is a doctor in a well-known tertiary hospital in China, seeing outpatient clinics, conducting research, meetings, applying for projects …… always has a myriad of work to do. “Sometimes the New Year holidays can not be a good vacation, a phone call will call him away …… last year we had a child, but basically I am taking. Every day when he comes home, the child is already asleep. I’m really worried that when the child grows up again, his father will have no time to play with him.” Nonetheless, she said she understands her husband. “He is so busy, but also the nature of the work, because the work of the doctor is to save lives ah, of course I have to fully support.” However, fund manager Xiao Song thinks otherwise. He said he knows several female friends who are doctors, they are busy every day, “after marriage, they have time to do housework and bring up children? My work is also very busy, really want to get married, but also to find a family-friendly.” There are also netizens who think of doctors as too scary: “Doctors are in contact with patients and germs all day long, and to operate on people, I think it’s a little scary to be with them.” A tertiary hospital female doctor Lu Yan (a pseudonym) is almost 30 years old this year, but still alone, “I spend most of the time in the hospital, it is easy to get off work, just want to go home and have a good sleep, who has the energy to make friends in society?” All this seems to be far from the situation 20 or 30 years ago, when the profession of “doctor” was a “plus” for people in selecting a marriage partner. “At that time, I thought that doctors were learned and respected.” An old man in his 60s said. Finally, there is pressure from society. In the “reluctance” to marry a doctor, a significant proportion of people give the reason is that doctors “always scolded by patients. In this regard, Hong Tao has a deep understanding. “We would like to help patients to relieve pain, but we are not God, indeed there may be treatment effect is not as good as the patient expected. In case the treatment is really not cured, some patients or patients’ families will say some very strong words, or even hit the doctor.” In a recent survey by China Youth Daily and the Clove website, 60 percent of the doctors surveyed said they had personally experienced or seen their colleagues being beaten. Doctors are facing “fewer successors”, “less money”, “no time for family”, “always scolded by patients”. What are the possible consequences of these evaluations of doctors by the general public? In the survey, 66.09% of the respondents said “no” to the question “Are you willing to be a doctor yourself? When asked if they support their children to become doctors, 60.52% of the respondents were clearly against it, while only 15.77% supported it. “Why do you want to be a doctor?” Xiao Chen said, “It’s risky, less money and less leisure time.” A 16-year-old high school student, on the other hand, said, “What if I accidentally prescribe the wrong medicine to someone, how serious the consequences would be. It’s so stressful, I can’t even think about it.” “If you ask doctors these questions, I’m afraid there are more who say ‘no’.” Hong Tao said helplessly, “After all, the general public has not personally experienced the situation of doctors. Wage problems, no time for family, being scolded is certainly one of the reasons, the work is not trusted, not respected, but more heartbreaking.” “This issue is well worth our attention.” Zhang Wei stressed. If the community at large does not want to be doctors themselves, or their own family members, the consequences are worrying. “We have to try to think about where the next generation of doctors will come from. If people in the future get sick, who will see them? It’s true that there will definitely be doctors. But if fewer and fewer people want to become doctors, will the number of good doctors decrease in the future as well? That would be a great loss to our society.” In addition, even at present, the people’s poor evaluation and distrust of doctors affects the harmony of doctor-patient relationship and the motivation of doctors’ work. Li Benfu, director of the ethics branch of the Chinese Medical Association, said that this will even increase the burden of “expensive” patient care. “In the past, when doctors saw patients, they did what was right for them; but now, because some patients don’t trust doctors, they have to take ‘defensive treatment’ in the treatment process. As soon as the patient enters the door, let him check from head to toe again, the cost is high, the patient complains naturally more. But in reality, it’s a way for doctors to protect themselves.” But when patients spend more money, they inevitably complain about the doctor, which in turn will further lower their evaluation of the doctor. In this way, doctors and patients are caught in a vicious circle, the people’s quality of medical care has not been improved as it should be, but rather derived from new conflicts. And all this is what we do not want to see. How to look at doctors objectively and fairly Want to reshape the good image of doctors, let people look at doctors objectively, first of all, we have to find the “cause of the disease”. The reason for the decline in the status of doctors in the eyes of the people, Li Benfu analysis, this is first due to individual medical staff medical ethics, medical style decline. “In the past, we have been advocating people-oriented, but under the huge economic temptation, a considerable part of the people may be ‘interest-based, people at the end’.” Xia Xueluan said, “In the ranks of doctors, there is also the phenomenon of receiving red packets.” With the gradual increase of similar situations, the image of doctors is naturally greatly diminished. Second, the medical and health care system does not adapt to the development of society. Li Benfu pointed out that there are governmental factors as well as problems in hospital management. For example, in some places, the hospital not only to earn money to support themselves, but also to feed some functional departments, in such an environment, the hospital, doctors will have to “talk business”. Therefore, the government should new rectification of the medical team, strengthen the training of medical ethics, and establish a set of doctors to “rely on technology rather than rebates to eat” mechanism, and therefore, will be submitted to the National People’s Congress at the two sessions this year to consider the final program of medical reform has also received wide attention from all walks of life. The people should also look at the current situation of doctors’ survival objectively and understand their difficulties. A netizen said well: “If I were sitting in the position of a doctor, I would probably be forced by life to receive some red packets and rebates; will also be impatient because they have to see too many patients every day; will also be for the sake of insurance, and more patients to do a few tests. Whenever I think of this, I can understand the doctors. Since it is not easy for all of us, why can’t we each be patient and restrain ourselves? This is also good for the medical environment and the patients themselves.” Li Benfu also said that, according to his understanding, many people are just not satisfied with the general environment of the entire health care industry, or should be viewed differently from the specific work of medical personnel. Xia Xueluan finally called on society should give doctors a fair and just public opinion environment. Only mutual understanding and tolerance between doctors and patients, our medical and health care can develop in a healthy way, the people’s health is guaranteed.
