What is chronic atrophic gastritis

1, complex disease overview “complex” is the main trunk of the meridian branches, “Ling Shu? The “pulse degree” says “the meridian is the lane, the branch and the horizontal one is the luo, and the other one of the luo is the sun”. It is widely distributed among the tissues of the internal organs, forming a network structure. The internal organs and the external limbs and muscles are intertwined and interconnected. It has the functions of communicating with the internal and external surfaces, penetrating the Qi and blood of Ying and Wei, penetrating the blood and fluid of each other, and moistening the tissues of the internal organs. As the ligament is the pathway for the transmission and circulation of Qi and blood and fluids, and the ligament body is small and widely distributed, once the ligament is blocked by Qi, blood and phlegm, the flow of Qi and blood and the transmission of fluids in the ligament will be affected, resulting in the ligament not being open or not being infiltrated properly, thus leading to basic pathological changes such as stasis of blood blocking the ligament, dystrophy of the ligament, and deficiency of the ligament not being honored, which results in ligament disease. The theory of loxopathy started from Nei Jing, and the clinical evidence and treatment was based on Jin Kui Yao, and the important pathological theory was formed by Ye Tian Shi in Qing Dynasty. Chronic atrophic gastritis is a common and difficult disease in gastroenterology, and clinical doctors have different opinions on its pathogenesis. The author’s mentor, Zhou Bin, based on its long duration of illness, mixed pathogenesis, and interchangeable evidence, according to Ye’s “clinical guide to medical cases” clearly put forward the “long-term disease into the luo”, “long-term pain into the luo”, and “the disease after several years, the In addition, according to Ye’s discussion of “liver yang directly offending the stomach luo”, and combined with the pathological characteristics of atrophic gastritis, which is often characterized by the role of emotional and psychological factors, atrophic gastritis is classified as a luo disease in Chinese medicine. The underlying pathogenesis of atrophic gastritis is based on the weakness of the spleen and stomach, the evil of cold and dampness, and the deep ambush of blood in the stomach ligaments, and the fear of cold. The pathological products such as Qi stagnation, phlegm coagulation and blood stasis are produced, which eventually lead to the deficiency of cold in the stomach and stomach ligaments, loss of warmth, dystonia in the ligaments, stagnation and obstruction, and pain if they do not pass; deficiency of Qi and blood in the stomach and stomach ligaments, loss of glory in the ligaments, and pain if they do not glory, resulting in the pathological process of stomach and stomach ligament disease. In other words, “weakness of the spleen and stomach, the evil of cold and dampness, deep in the stomach complex” is an important cause of chronic atrophic gastritis and the underlying pathological mechanism. The body’s qi, blood and fluids run on the warmth of Yang Qi to promote the smooth flow of unimpeded. If the cold evil is directly in the lining and injures the spleen and stomach, the qi and blood of the meridians will lose the warmth of yang qi, which will easily cause the qi and blood to condense and stagnate, and the pain will be caused by astringency and stagnation. Su Wen? Pain is said: “The cold gas guest between the stomach and intestines, under the membrane original, the blood can not be dispersed, the small complex leads to urgent, so pain.” The spleen is a yin earth, like dryness and hate moisture, moisture is a yin evil, easy to trap the spleen Yang. The spleen is trapped by dampness, transport and transformation is not strong, diet water and grain can not be transformed into gas and blood essence. Water in turn for wet, grain in turn for stagnation, wet stagnation for a long time, and thus heat, can lead to heat depression in the center, damage to the stomach complex. When dampness is trapped for a long time, spleen qi deficiency, qi deficiency and yang, yang deficiency and cold, the blood is not warmed, and it becomes stagnant; spleen qi deficiency, qi does not move blood, the blood stagnates; spleen loss of control, blood does not follow the meridian, overflowing outside the veins, the blood away from the meridian for a long time does not dissipate, stagnant and becomes stasis. In the Treatise on the Origin of Diseases, it is said that “blood stagnation and blood stasis are also caused by congestion and obstruction”. Therefore, stagnation of qi and blood stasis is an inevitable pathological process of gastric gangrene. Stagnation of the gastric complex, gas does not distribute fluid, blood does not nourish the meridian, the stomach loses nourishment and glory, the stomach is damaged, the glands are atrophied, the mucosa becomes thin and becomes atrophied over time. Modern clinical medicine classifies chronic atrophic gastritis as gastric gangrene, and its gastroscopy shows that the gastric mucosa loses its normal color, red and white, mainly white; the mucosa becomes thinner, the folds become flat or even disappear, and the blood vessels are exposed, which is caused by the stagnation of the gastric veins and ligaments, the blood flow is not smooth, and the stomach loses its moistening. Some chronic atrophic gastritis can be observed microscopically in the form of granular or nodular mucosa, which, according to Wang Qingren’s view that “those who have nodules must have visible blood”, is due to blood stasis. In summary, tutor Zhou Bin believes that chronic atrophic gastritis is a disease of both blood and qi, with blood being the main cause of the disease. The external cold and dampness of the evil guest in the muscle surface, stay and not go, into the meridians, into the stomach, or cold and dampness of the evil directly in the internal organs of the meridians; or the host body spleen and stomach weak, over-eating cold diet, cold and dampness of the evil stays in the stomach, through the years. The evil stays, the disease is long into the complex, the stomach complex stasis, stagnant blood stasis, do not pass is pain; stomach complex stasis, the deficiency of gas complex, stomach loss of nourishment rong nourishment, “not rong is pain”. Atrophic gastritis should belong to the category of Chinese medicine luo disease, to “spleen and stomach weakness, the evil of cold and damp, deep ambush stomach luo” as the fundamental pathogenesis of chronic atrophic gastritis. 4, atrophic gastritis complex pathology treatment Ye Tianshi believes that “where the complex is deficient, the most appropriate to pass tonic”, advocating “qi deficiency, then tonic in order to move qi, blood failure, then nourish the camp to pass complex”; and proposed “complex to pungent for drainage “The theory of chronic atrophic gastritis is a good guide for the treatment of chronic atrophic gastritis. According to the fundamental pathogenesis of atrophic gastritis, “the spleen and stomach are weak, and the evil of cold and dampness is deep-rooted in the gastric ligaments”, clinically, the instructor mostly adopts the treatment principle of benefiting the qi and strengthening the spleen, warming the cold and dampness, and pungent and warming the ligaments. The instructor often use the four gentlemen’s soup plus flavor treatment, the medicine with the party ginseng 15g, Poria 20g, bran fried white atractylodes 15g, the French half of the summer 9g, Gan Song 10g, dry ginger 6g, Danshen 20g, pregis 15g, sorrel 15g, Gui Zhi 6g, raw astragalus 20g, fried white peony 15g, sand 6g, honey licorice 6g. in the party ginseng, white atractylodes benefit qi and strengthen the spleen; Poria to benefit the spleen and stomach, and can percolate water and damp, with Astragalus, can increase the function of strengthening the spleen and Qi; Fa Hanxia pungent and warm dry, can dry dampness and phlegm, and stomach and vomiting; Sharen pungent and warm, can transform dampness and stomach, warming the spleen; Gui Zhi pungent and warm, warming the meridian, help Yang Qi, dispersing cold and pain, and Bai Zhu, Poria, licorice used together to warm the spleen Yang, dampness and water; Gan Song, dry ginger, pungent and warm, warming the cold; Danshen blood stasis, dredge the meridians; White peony nourishes blood and meridians, alleviate pain Prepared by the Chinese medicine; Prepared by the Chinese medicine; Prepared by the Chinese medicine; Prepared by the Chinese medicine; Prepared by the Chinese medicine; Prepared by the Chinese medicine; Prepared by the Chinese medicine; Prepared by the Chinese medicine The whole formula can benefit Qi, strengthen the spleen, warm the middle and relieve pain, invigorate blood circulation, resolve dampness and harmonize the stomach, and has the characteristics of taking care of both the symptoms and the root cause. The spleen is the master of transportation and transformation, and is the essence of the latter, preferring dryness to dampness. In addition, Chinese medicine believes that “where evil comes together, its qi must be deficient” and “when positive qi exists within, evil cannot dry out”, so we use the method of strengthening the spleen. The main reason for this is that it is a good idea to have a good understanding of the situation. The chronic atrophic gastritis is mostly due to cold and dampness into the complex, non-warm, the cold does not disperse, non-passing blood stasis does not dissolve, to pungent warmth to pass the complex as a method to pass blood stasis and disperse cold, so that the gastric complex stagnation can be carried out, the qi flow is smooth, the evil to go and is safe, the qi and blood reconciliation to get through, through the disease healed. 5.Clinical case example Liu, female, 60 years old, was first diagnosed on May 9, 2011. On January 14, 2011, the pathological diagnosis of gastroscopy was moderate chronic atrophic gastritis of the mucosal tissue of the gastric sinus. The patient has gastric distention and fullness, especially after eating, poor appetite, vague pain in the stomach, warmth and pressure, noisy stomach, occasional belching, and bowel movement. The tongue is light and dark, with white greasy coating and thin string pulse. Herb: Radix et Rhizoma Atractylodis Macrocephalae 15g, Atractylodes Macrocephalae 15g, Poria 20g, Fructus Semen 9g, Fructus Citrus Aurantium 15g, Radix et Rhizoma Glycyrrhiza 20g, Radix et Rhizoma Ginger 10g, Radix Salviae Sinensis 20g, Radix et Rhizoma Paeoniae 20g, Radix et Rhizoma Chai Hu 10g, Radix et Rhizoma Aromaticus 20g, Radix et Rhizoma Ginger 10g, Radix et Rhizoma Perilla 20g, Radix et Rhizoma Salviae Sinensis 20g, Radix et Rhizoma Sand 6g. The pain disappeared, and the greasy coating improved compared with before. After taking the medicine, the symptoms disappeared after 3 months, and the gastroscopy was repeated on August 2, 2011. After six months of follow-up, the patient’s symptoms were stable without recurrence. 6.Summary Supervisor Zhou Bin has been specialized in gastroenterology for more than 20 years, and has a lot of experience in the treatment of chronic atrophic gastritis. He concluded that chronic atrophic gastritis is caused by spleen deficiency, external sensation of cold and dampness, from qi and blood, from actual to deficiency, from deficiency to stasis, phlegm and stasis, and stasis of the ligaments, thus forming a stomach ligament disease. The understanding of chronic atrophic gastritis from the perspective of luo disease and the pathological mechanism of “cold and dampness entering the luo” is of great significance to the clinical treatment of this disease.
