Abstract: Cervical erosion is not pathologically true erosion due to epithelial ulceration and loss. It is generally referred to as cervical columnar epithelial ectasia in clinical practice. The patient in this case is a 47-year-old woman who came to our hospital with bleeding after intercourse. The patient was diagnosed with cervical erosion after performing relevant tests and was given medication and physical therapy for her current condition. After 10 days of treatment, the patient’s discomfort has basically disappeared, but she still needs to be followed up regularly to minimize the recurrence rate.
Basic information】Female, 47 years old
Disease Type】Celiac disease
Hospital】Hunan Provincial Maternal and Child Health Hospital
Date of consultation】May 2022
Treatment plan】Medication (cefixime dispersible tablets, compound apricot fragrance rabbit ear wind capsule, Cihang capsule) + physical therapy (cryotherapy)
Treatment period】1 day of outpatient treatment, 10 days of out-of-hospital medication
Treatment effect] The disease was controlled
I. Initial consultation
The patient was 47 years old, she had a cesarean section in 2005, her last menstrual period was on April 25, she had sexual intercourse, and she had an IUD for contraception, she came to our hospital because of bleeding after intercourse, and no special treatment was given before the visit. The patient complained of bleeding after intercourse 3 days ago, but the amount of bleeding was small, and the bleeding was white with blood.
Second, the treatment process
Generally, cervical erosion is a normal physiological state and does not require treatment. However, because the patient also had chronic cervicitis, the patient was advised to treat the chronic cervicitis. The patient finally decided to adopt a comprehensive treatment approach, which means using medication for chronic cervicitis, including cefixime dispersible tablets to play an anti-inflammatory role; compound apricot fragrance rabbit ear wind capsule and cichlid capsule to play a role in clearing heat and detoxification. Then the physical treatment method of cryotherapy is used in the outpatient clinic to induce necrosis and detachment of the inflammatory surface and repair the cervical trauma.
III. Treatment effect
After 10 days of medication, the patient was seen again in our outpatient clinic, complaining of no obvious adverse reactions during the medication period, and the color of the discharge changed from yellowish to translucent. Gynecological examination at the outpatient clinic showed that there was no obvious abnormality in the vagina, the cervical erosion surface basically disappeared, and there was no touching of blood in the cervix, and no obvious abnormality in the TCT examination.
IV. Precautions
The patient was basically cured after 10 days of treatment and was in a good mood, and I was very happy for the patient’s recovery. I also reminded the patient that although the disease has been basically cured, but because of the previous cervical biopsy, it is recommended to have intercourse after half a month, when the results of the follow-up examination indicate that the cervix has recovered; and it is not recommended to go to public pools, hot springs, etc., for underwater activities after the treatment to avoid causing cervical inflammation and cervical erosion again. In addition, if bleeding after intercourse, abdominal pain and abnormal leucorrhea occur again after the end of treatment, prompt medical consultation should be made to reduce its recurrence.
V. Personal insight
If it is only physiological cervical erosion, also known as cervical columnar epithelial ectopic, no special treatment is needed. However, if, like the patient in this case, it is pathological cervical erosion caused by diseases such as chronic cervicitis, it requires active symptomatic and causal treatment to restore the normal form of the cervical mucosa as soon as possible, otherwise it may lead to increased damage to the cervical mucosa and even cause abnormalities in the uterus and other reproductive systems.
