Traditional treatment of tumor includes surgery, chemotherapy, radiotherapy and Chinese medicine treatment, etc. Tumor interventional therapy has the advantages of minimally invasive, low cost, safety and good efficacy, etc. Especially for those tumor patients who cannot be operated, tumor interventional therapy is showing its status in tumor treatment more and more. Current status of tumor interventional therapy: Tumor interventional therapy can be divided into two categories: transvascular and non-transvascular. Transvascular tumor interventions include: arterial perfusion chemotherapy and arterial embolization. Non-transvascular tumor interventional therapy can be roughly divided into: 1. percutaneous perforation (or non-perforation) therapy. Such as percutaneous intra-tumor injection of anhydrous alcohol, dilute hydrochloric acid or other chemotherapeutic drugs; percutaneous puncture for radiofrequency ablation, microwave ablation, argon helium knife treatment, laser ablation of tumor. Percutaneous HIFU knife (ultrasound concentrator knife) treatment.2. Endoscopic resection or ablation of tumor, or injection drug treatment, etc. Different methods of tumor interventional therapy have their own advantages and disadvantages: 1. Arterial infusion chemotherapy has the advantages of high concentration of local chemotherapeutic drugs for tumor and less systemic toxic side effects than intravenous chemotherapy. However, for tumors of substantial organs, simple infusion chemotherapy is far less effective than arterial infusion chemotherapy combined with embolization therapy. 2.Arterial embolization therapy has greatly improved the efficacy of solid tumors such as liver cancer, but for cavity organs such as intestinal cancer and bladder cancer, in principle, embolization therapy is not recommended to avoid complications. The biggest problem now is the recanalization and regeneration of blood vessels after embolization. After embolization, the imaging shows that the tumor vessels are completely filled with embolic agent, which seems to have cut off the tumor blood supply, but when the second re-imaging is done, it is often found that the vessels in the tumor area are reopened or partially reopened, or the original embolized vessels are not reopened, but new tumor vessels appear around the original tumor, suggesting that the tumor is still growing or recurring, so the arterial embolization treatment must be done at least twice. 3.The direct killing of tumor through puncture or under endoscope, no matter using heat (such as laser, radio frequency, microwave or ultrasonic knife), cold (argon helium knife) or chemical methods (anhydrous ethanol, dilute hydrochloric acid) can achieve more accurate efficacy, but there are still many shortcomings, such as radio frequency ablation or ultrasonic knife generally need to be guided by B ultrasound, which is related to the level of B ultrasound physician and also its own sensitivity. This is not only related to the level of the ultrasound physician, but also to the sensitivity of the patient. Even if CT-guided puncture injection drug treatment is performed, it is only suitable for lesions that can be displayed by CT, but not for lesions of equal density with normal tissues, and the relationship between the dose of injected drug and tumor size lacks standardized protocol. In addition, the puncture needles currently used for radiofrequency or argon helium knife treatment are still relatively thick, which can damage normal organs and also easily lead to hemorrhage if the lesions are located near the edges of organs or large blood vessels. The shape of electrode is not perfectly matched with the shape of lesion, which needs to be improved. Redevelopment of interventional tumor treatment Currently, interventional medicine uses robots to locate and puncture tumors through real-time visualized CT (64, 128-row CT), which is faster and more accurate than the doctor’s own puncture and also avoids damage to normal tissues. Interventional treatment under open MRI avoids the effects of radiation on patients and medical personnel, and avoids errors in CT scanning of isointense tissue. Microcatheters with laser treatment systems or catheterization systems with ablation avoid the damage of percutaneous puncture. A puncture needle system that fits perfectly to the lesion will improve the efficacy of radiofrequency ablation and argon helium knife therapy. Gene interventional therapy for tumors is another new direction and trend, because tumors are fundamentally genetic diseases and treatment at the genetic level may control tumors at the root. Conventional chemotherapy and radiotherapy do not correct the intrinsic factors that cause tumor development, even if the tumor cells found are eliminated, new tumor lesions will grow out in another area after a while, which is a difficult point to be solved in tumor treatment. Gene therapy directly introduces normal oncogenes into tumor cells with the help of interventional technology to replace the problematic genes for the purpose of cure, which becomes a new type of tumor treatment at molecular gene level after surgery, radiotherapy and chemotherapy. In addition, molecular drugs are delivered directly to tumor local slow release by using interventional technology to achieve inhibition of tumor cell growth, induction of tumor cell apoptosis, or inhibition of tumor vascular growth. We combined vascular endothelial inhibitory factor and interventional embolization technology to solve the problem of tumor vascular regeneration after interventional embolization. Interventional gene therapy for tumor has the characteristics of good efficacy, less side effects and strong targeting compared with conventional chemotherapeutic drugs, which can better play the role of specifically killing tumor cells and not affecting normal cells as much as possible. Comment: At present, the best treatment effect of tumor is the combination of Chinese and Western medicine, western medicine is relatively direct and fast, but the side effects of western medicine should not be ignored. The best treatment effect can be achieved by combining Chinese and Western medicine.
