What is perianal abscess

  I. What is perianal abscess?  Perianal abscess, known as anal canker sores in Chinese medicine. It is an acute purulent infectious disease that occurs around the anus, anal canal and rectum, which is a bacterial infection and is the predecessor of anal fistula. The incidence of perianal abscess is higher in men than in women and can occur at any age, with the peak age of onset usually being 20-40 years. The clinical manifestations are rapid onset, severe perianal pain, intermittent or persistent, and some patients with generalized high fever.  Second, what are the triggering factors of perianal abscess?  According to Chinese medicine, this disease is mostly caused by over-eating fatty, sweet, greasy and spicy products, resulting in dampness and heat injecting into the large intestine and accumulating in the anus; or by trauma to the anus, invasion of evil toxins, obstruction of the meridians, and stagnation of qi and blood. It is also caused by deficiencies in the lungs, spleen and kidneys, and damp-heat is injected into the anus due to deficiency.  Modern medicine has found that most perianorectal abscesses have foci of infection from anal gland infection. It can also be secondary to anal fissures, proctitis, ruptured thrombosed external hemorrhoids, and trauma. In addition, systemic diseases such as diabetes mellitus, blood disease, malnutrition, and overexertion lead to low resistance to infection, which can also easily induce perianal abscesses.  How to treat and prevent perianal abscess?  At present, the treatment principle of perianal abscess is timely incision and drainage. Generally, after the abscess is drained, the patient’s pain can be relieved immediately. After surgery, intravenous antibiotics are given to fight infection, and oral herbal tonics are given to clear heat and detoxify the toxins.  In about 70% of patients with perianal abscess, an anal fistula will form after drainage. Therefore, it is generally recommended that patients with post-operative abscesses should be followed up in outpatient clinics to confirm the formation of anal fistula with the help of imaging methods such as ultrasound or MRI, and once the fistula is formed, subsequent radical surgery should be actively performed to avoid recurrent attacks of perianal abscesses that may affect daily life and work.  In the usual life, you should also do a light diet, avoid spicy, tobacco and alcohol stimulation, avoid diarrhea, pay attention to rest. And actively treat primary anorectal diseases such as anal fissure, proctitis and thrombosed external hemorrhoids! Once there is perianal discomfort, pain, or accompanied by fever, you should seek medical attention as soon as possible, do not delay treatment and aggravate the development of the disease.
