After combining symptoms, physical examination and imaging, he was diagnosed with acute cerebral infarction induced isotropic hemianopia. As the patient requested conservative treatment, he was given aspirin enteric tablets and butylphthalide soft capsules for treatment. After 14 days of hospitalization, the patient recovered his visual field and his quality of life improved significantly. Physical examination: clear consciousness, fluent speech, symmetrical bilateral eyelids without ptosis, adequate bilateral eye movements in all directions, no nystagmus, complete right-sided isotropic hemianopia in the nasal side of the left eye and temporal side of the right eye, symmetrical bilateral frontal lines and nasolabial folds, central tongue extension, soft neck, muscle strength of the extremities grade V, slightly hyperalgesia in the right extremity, normal muscle tone, negative double baroreflex signs. Magnetic resonance scanning of the head and DWl suggested multiple recent infarcts in the cerebellar hemispheres, left thalamus and left temporoparietal-occipital lobe. It was determined that acute cerebral infarction induced isotropic hemianopia. The patient came to the clinic 10 hours after the onset of the disease and missed the thrombolysis time, and the brain surgery consultation was not suitable for interventional thrombolysis treatment. The family refused further examination and requested conservative medical treatment, so they were given aspirin enteric solution tablets to antiplatelet aggregation, atorvastatin calcium tablets to intensify lipid lowering and stabilize arterial plaque, butylphthalide soft capsules to promote collateral circulation, injectable thromboxane to improve ischemia, methylcobalamin tablets to nourish peripheral nerves and acupuncture rehabilitation. IV. Precautions We are glad that the patient’s condition was controlled after the treatment. After discharge, the patient should maintain a low salt and low fat diet, monitor blood pressure and blood glucose regularly so that blood pressure is ≤140/90mmHg, fasting blood glucose is 6-7mmol/L and 2 hours postprandial blood glucose is 8-10mmol/L.
