Chronic shoulder joint pain is not the same as “frozen shoulder”

  Ms. Hao, 45 years old, likes to play table tennis. In the past six months, she had recurrent pain in the front and upper part of her right shoulder, and her right hand could not be lifted, which affected her life. Later, she went to Zhongshan Hospital for MR examination and found that Ms. Hao had a right rotator cuff injury and a large amount of subacromial bone formation. She was diagnosed with subacromial impingement. She underwent arthroscopic subacromial impingement and resumed playing table tennis shortly after the surgery.  High prevalence of shoulder pain: In addition to the elderly, fans of badminton, table tennis and baseball sports.  Along with the accelerated pace of life and the aging process of the population, many middle-aged and elderly people in life suffer from shoulder joint pain and stiffness. Also the popularity of sports has caused recurrent chronic shoulder pain in some young people, especially those sports enthusiasts and athletes who are engaged in shoulder movement training, such as badminton, table tennis, and baseball enthusiasts. At present, most people have a lack of knowledge about shoulder pain, and many people are used to generalize all shoulder pain to “frozen shoulder”. Some people do not take shoulder pain seriously and still perform inappropriate exercises, such as hand-carrying, heavy lifting, or intense badminton and other abduction exercises, thinking that they will get better if they move more and stretch out their muscles and bones, but it is often not what they want and their symptoms gradually worsen.  Find out the cause: symptomatic treatment is crucial.  What causes shoulder joint pain? In fact, there are many causes of shoulder pain, and different causes of shoulder pain have different manifestations and characteristics, so it is crucial to make a clear diagnosis as soon as possible and find the cause for targeted treatment. For example, shoulder pain secondary to cervical spondylosis is often accompanied by numbness in the hands and radiating pain in the upper extremities, so the treatment should mainly focus on the primary cause – cervical spondylosis; shoulder pain caused by diabetic patients or prolonged shoulder braking after trauma is often accompanied by significant limitation of shoulder joint movement in all directions, which is caused by adhesions of the shoulder capsule, so it is also called adhesive capsulitis, frozen shoulder, etc. The treatment should mainly focus on the shoulder capsule. Therefore, some patients can improve the function of the shoulder joint through rehabilitation exercises such as rope exercises, ring pulling and stick exercises under the guidance of a professional doctor. Bursitis is caused by inflammation of the subacromial bursa of the scapula (a bony structure above the scapula) due to repeated mechanical stimulation. The pain is mainly located in the anterior and superior part of the shoulder joint. Arthroscopic acromioplasty can be used to achieve satisfactory results, but delayed treatment can often result in secondary tendon damage, which can seriously affect the patient’s function and life. In such cases, there is often damage or rupture of the joint capsule, ligaments and tendons in the corresponding areas. Minimally invasive arthroscopic surgery to repair the corresponding damaged structures is currently the “gold standard” for the treatment of many sports injuries, enabling patients to return to the sports field.
