Surgical treatment of microphthalmia in children

   Congenital microphthalmia and anophthalmia are rare clinical abnormalities of ocular development. It is usually combined with other ocular developmental abnormalities. It is often an ocular manifestation of a generalized syndrome. The incidence of congenital microphthalmia is 1.5-19/10,000, and the incidence of congenital anophthalmia is 0.18-0.4/10,000, in which the basic structures of the eye cannot be observed by visual or imaging examination. congenital microphthalmia and anophthalmia may cause abnormalities in the development of the affected orbit and face.  Currently, the treatment for congenital microphthalmos and anophthalmos is very outdated in China. The general approach is to remove the aphakic microphthalmia and implant a prosthetic table in 1 or 2 stages. In the case of anophthalmia, the prosthesis is implanted directly. The constant size of the prosthetic table is not compatible with the growth pattern of the eye and therefore cannot adequately stimulate orbital development. A large number of clinical and experimental studies abroad have demonstrated that the effect of prosthetic tables on stimulating orbital and facial development is questionable. At the same time, it is believed abroad that even if the child’s eye is small, early eye removal surgery cannot be performed. This is because the small orbital orbits interact with each other and their presence facilitates the normal development of the orbits.  There is a set of advanced treatment methods abroad for different cases of congenital microphthalmia and anophthalmia. Through the cooperation with Aier Yingzhi Eye Hospital, we have introduced foreign advanced surgical techniques and surgical materials to develop a set of specific treatment methods suitable for children in China. The specific treatments will be introduced to you one after another in future articles.  In the treatment of congenital microphthalmia and anophthalmia, the standardized production and reasonable wearing of prosthetic eyes are particularly important. Therefore, we have cooperated with the most famous Liying Activity Prosthetic Eye Center in China to make and wear prosthetic eyes for many children with eye extraction and congenital microphthalmia and anophthalmia. Not only have we achieved excellent treatment goals, but we have also solved the cosmetic problems that plague these children.  Microphthalmia, also known as microphthalmia, is a congenital genetic disorder in which the eyelids are extremely small and the distance between the inner and outer canthus is extremely short, with the typical tetralogy of ptosis, narrow lid fissures, inverted medial canthus, and widened medial canthus spacing. It can affect the child’s appearance, visual development, and mental health. In Beijing Children’s Hospital, the surgical correction of microphthalmos, entropion, and ptosis has yielded good results, improving the child’s facial appearance and promoting normal visual and psychological development.  1. Shan Shan, a girl, is the youngest child with small lid surgery in our department, whose eyes could not be opened after birth and were thin. Because of her severe hyperopia and astigmatism, she underwent blepharoplasty at the age of 1.5 years for early treatment of amblyopia. 2, Xinxin, girl, 3 years old, small eyelid fissure syndrome, because of the small, slit-shaped eyes after birth, whenever the children and toys to play games, because the eyes can not open the head always open mouth struggling to see, and from time to time have to use both hands to lift the upper eyelids, hands can not be released as a result of grabbing toys, always anxious to cry, children also do not like to play with her, the child therefore Before the child was hospitalized in our department for plastic surgery in October 2009, we were impressed to see these special movements of the child every time we visited the hospital. Now, two months after the surgery, the child’s eyes have become bigger and longer, the eyelids are lifted, and the child’s outlook has been surprisingly improved. After the surgery, the mother shed tears of excitement: “I didn’t expect my baby to be so beautiful! What a difference!”  3.Chen Chen, a boy, 3 years old, was recognized as a “handsome boy” in our ophthalmology ward after his successful blepharoplasty surgery, and was really loved by everyone.  4. Lele, a girl of 10 and a half years old, has bilateral microphthalmos combined with various ocular developmental abnormalities: upper and lower eyelid ectropion, strabismus, and retinal detachment in the left eye, and has undergone strabismus correction and multiple vitrectomy combined with retinal repositioning in the left eye. Because of her poor vision and special appearance, Wen had become extremely sensitive and had low self-esteem, and she hated looking in the mirror. The picture shows one week after the successful microphthalmoplasty at our hospital. Both the child and the parents were very happy with the improvement in appearance. Wen, who loves beauty, is now willing to look in the mirror!  5, Zhuang Zhuang is 7 years old, originally a lively and cute boy, but since he started school, he has become quiet, because he has microphthalmia, he is often nicknamed and joked by his classmates at school, and he has become more and more inferior. After a successful blepharoplasty at our hospital, the boy’s chubby face was restored to its usual lovely smile.
