”After the fracture heals, must the plate screws that hold the fracture in place be removed again?” Many patients and their families ask this question after undergoing orthopedic internal fixation surgery. Many patients and their families even take the answer to this question as the reference standard for choosing internal fixation devices: if they want to remove it later, they should use cheap domestic plates; if they don’t need to remove it later, they should use imported plates. In fact, this understanding is not correct. With the progress of raw material industry and the development of orthopedic internal fixation instruments, the biocompatibility and stiffness of the orthopedic internal fixation plate screws applied in most medical institutions are quite good. The so-called material difference between domestic and imported plate screws is a kind of statistical theoretical difference, for example, the load fatigue test with imported plate screws and domestic plate screws under the same conditions, only 1 piece of imported plate breaks in 1000 pieces, while 6 pieces of domestic plate break in 1000 pieces, but in fact, the proportion of breakage in clinical application of domestic plate is still very low. Even for imported plates, if the fracture does not heal, the plate will eventually loosen or fracture. As for the material of the steel plate, the quality of some domestic steel plate does have some defects, more common is the stress weak point at the connection of the screw tail and screw rod, sometimes there will be an attempt to tighten the screw, the fracture occurs at the connection of the screw tail and screw. Or the screw tail slips and slips and cannot be screwed with a screwdriver. If this happens when the steel plate is removed, it will cause a lot of trouble, so the idea that “if you want to remove it later, you should apply domestic steel plate” is not right. In fact, for adults, regardless of the choice of domestic or imported steel plates, most of the plate screws are not necessary to remove. Unless the body rejects the plate screw, the incision does not heal for a long time, oozing, or the plate screw loosens, or the plate screw bulges under the skin and irritates the skin or affects the sliding of the tendon, then consider removing the plate screw after the fracture heals.
