What dilating eye drops should I use during my optometry exam?

  When parents take their children to the ophthalmologist for an eye exam, the doctor often requests a dilated eye exam. Children of different ages and conditions require different dilating eye drops.  For preschoolers, they usually need atropine eye drops or ointment, which is usually ordered three times a day, one drop in each eye, for three consecutive days, before going to the hospital on the fourth day for an eye exam. Atropine is a long-acting pupil-diluting drug, the advantage is that it can completely paralyze the ciliary muscle, the most accurate optometry results, reflecting the child’s true refractive state; the disadvantage is that the drug metabolism is slow, up to 14 days, during which the child will be afraid of light, can not see the word near, affecting children’s learning and play, if absorbed through the nasal tear duct through the nose, will lead to eye redness, dry mouth, fever and other side effects. Therefore, after using atropine to spot the eye, it is important to compress the inner corner of the eye for 3-5 minutes to prevent the side effects of atropine absorption through the nose. The use of atropine for pupil dilatation is generally not recommended for school-age children during the school day.  For optometry in school-aged children and adolescents, short-acting dilating eye drops are usually used, such as Shuangxingming (tropicamide eye drops), Medolite (compound tropicamide eye drops), and post-matropine eye drops. Generally, the pupil is not dilated first, and then the pupil is dilated: the eye is dotted once every 5-10 minutes, at least 3 times in a row, and the pupil is dilated and the reaction to light basically disappears before the eye is tested. Among the above three drugs, Medrol has the fastest onset of action, followed by Shuang Xing Ming; it takes 5 hours for Medrol, 6 hours for Shuang Xing Ming, and more than 12 hours for Postmatropine to restore the pupil to the state in front of the dotted eye; the ciliary muscle paralysis effect is not as good as that of atropine, and there will be some residual adjustment power; except for Postmatropine, which does not cause dry mouth, the side effects of the above three drugs are comparable to those of atropine, but because of the short effect, the The degree will be much lighter. It is recommended to also compress the inner corner of the eye for 3-5 minutes after eye spotting.  For school-age children with combined internal strabismus or amblyopia, the doctor will also usually recommend atropine for the first eye exam.
