With the aging of the population, the unreasonable family structure and other issues coming to the fore, in 2013 the state launched the implementation of the “separate second child” policy. This is undoubtedly good news for families who want to have a second child, but most of the first only children in China are now over 35 years old, and they have already passed the peak of their childbearing years when they enjoy the “separate second child” policy, which is a high-risk pregnancy. The majority of these women are now over 35 years old and have already passed the peak of their childbearing years when they enjoy the “separate second child” policy. 1, make adequate pre-conception preparation Pre-conception preparation is particularly important for senior women. It is recommended to go to a regular hospital for a detailed pre-conception examination to make sure that all indicators are normal before preparing for pregnancy. Be physically and mentally healthy, quit smoking and drinking habits that have an impact on the outcome of pregnancy, avoid exposure to toxins and radiation, and those who use drug contraception should change to instrumental contraception and get pregnant after 6 months. Small doses of oral folic acid to prevent fetal neural tube malformation, multivitamins and minerals should be taken from 3 months before pregnancy. 2, regular and perfect prenatal checkups After successful conception, senior mothers must do their “homework” during pregnancy and have regular prenatal checkups. First of all, in early pregnancy, ultrasound should be performed to rule out ectopic pregnancy and to understand the development of the embryo in the uterus. Secondly, as the incidence of fetal chromosomal abnormalities increases in older pregnant women, the Implementation Measures of the Maternal and Child Health Law of the People’s Republic of China stipulates that pregnant women over the age of 35 should undergo prenatal diagnosis. In other words, amniocentesis is performed at 18-24 weeks of gestation for fetal chromosome examination. In addition, a fetal tertiary screening ultrasound should be performed at 20-24 weeks of pregnancy to rule out fetal structural developmental abnormalities. Older women are prone to pregnancy complications such as gestational hypertension and gestational diabetes during pregnancy. Therefore, it is important to monitor blood pressure frequently in the middle and late stages of pregnancy and pay attention to controlling salt intake; to do oral glucose tolerance test (OGTT) on time from 24 to 28 weeks of pregnancy to screen for gestational diabetes, to have balanced nutrition during pregnancy and to reduce high sugar intake. The main risks of advanced pregnancy The best age for women to give birth is 25-29 years old, and the medical term for pregnant women who are older than 35 years old when they are expected to give birth is advanced pregnancy. Compared with young pregnant women, the risk of senior pregnant women increases, mainly in the following aspects: 1. senior pregnant women are prone to spontaneous abortion As they age, ovarian function gradually decreases, and some senior women may have endocrine disorders, not easy to conceive, and once pregnant, they are prone to insufficient secretion of estrogen and progesterone or insufficient chorionic gonadotropin, and even pre-term miscarriage or fetal arrest. The pregnancy will be more difficult to conceive. The older the woman is, the longer the eggs are stored in the ovaries, the more chances of chromosomal aberrations and mutations due to pollution, radiation and drug accumulation, and the more likely it is that the meiosis process will cause chromosomal non-segregation, making it easier to have children with birth defects. The incidence of fetal chromosomes in the general population is about 1:800, the incidence of older pregnant women over 35 years old is 0.4%, and if the pregnant woman is over 40 years old, the risk rises to 1.4%. 3. Advanced pregnant women are prone to complications The vascular elasticity of advanced pregnant women is not as good as that of young pregnant women, which, together with the increased psychological burden, makes the risk rate of hypertensive diseases during pregnancy increase. At the same time, the ability of sugar metabolism of senior pregnant women decreases, and the risk of gestational diabetes is also higher than that of young pregnant women. 4, older pregnant women have a high rate of difficult and surgical deliveries After the age of 35, women’s physical strength is in decline, the pelvis and ligament function degenerates, the elasticity of the soft birth canal tissue is weakened, and the contraction force of the uterus is correspondingly weakened. In addition, the incidence of gigantic babies, excessive amniotic fluid, intrauterine growth restriction, intrauterine distress, etc. also increases, and the rate of obstructed labor and surgical delivery increases. Although there are many risks for senior pregnant women, they can still be happy mothers if they make adequate preparation for pregnancy, have a good prenatal checkup, maintain a good psychological state during pregnancy and increase their confidence.
