What are the chances of recurrence of meningioma?

  Many patients ask what are the chances of recurrence after meningioma surgery? How can recurrence be effectively prevented?  Grade I meningiomas (WHO I) account for 80% of all meningiomas. The average time to recurrence after Simpson’s grade I resection is 7.5 years, with recurrence rates of 5% at 5 years, 25% at 10 years, 32% at 15 years, and 37-55% at 20 years. Patients who cannot achieve primary resection have high recurrence rates, for example, meningioma recurrence rates of up to 85% with subtotal resection.  Grade II meningiomas (WHO II) account for 15-20% of meningiomas and have a recurrence rate of 40% at 5 years, even after primary resection, with an increasing recurrence rate over time. The mean time to recurrence is 2.4 years.  Grade III meningiomas (WHO III) account for 1-3% of meningiomas and are highly malignant meningiomas that recur relatively quickly even after primary resection plus radiotherapy, with a poor prognosis and an average survival time of 2 years.  Currently, there is no effective means to prevent recurrence other than surgery. Meningiomas that are not fully resected and grade III meningiomas can be treated with radiotherapy to delay recurrence.
