Puzzling joint pain caused by rheumatoid arthritis

  Joint pain is the most common symptom in rheumatology, and more than 80% of patients in outpatient clinics may be seen in rheumatology for joint pain; however, joint pain is also the most complex. Almost all rheumatologic diseases can present with joint pain symptoms, either arthritis with inflammation and redness, or simple, joint pain without inflammation or redness.  If you get joint pain symptoms, then you could have osteoarthritis, gout, or systemic lupus erythematosus, ankylosing spondylitis, or rheumatic fever, reactive arthritis, or psoriatic arthritis, dry syndrome, or vasculitis, scleroderma …… There are too many possibilities to say and explain. Rheumatoid arthritis is a systemic autoimmune disease with chronic, erosive inflammation of multiple joints throughout the body as the main manifestation. The incidence of the disease in China is about 0.32%-0.36%. There are currently more than 5 million patients with rheumatoid arthritis in China. This disease is one of the main causes of labor loss and disability in our population.  The disease and course of rheumatoid arthritis varies individually, from transient, mild oligoarthritis to acutely progressive polyarthritis. Joint pain is the most common symptom of rheumatoid arthritis, with the most commonly affected joints being the proximal interphalangeal joints, metacarpophalangeal joints, wrists, elbows, shoulders, knees and toes; the cervical spine, temporomandibular joints, sternoclavicular and acromioclavicular joints may also be involved, with impaired mobility. More than 95% of patients feel stiffness in and around their joints when they first wake up in the morning, and this symptom often lasts for more than an hour. The joint pain is accompanied by redness and swelling of the joints, and slowly the joints are destroyed, with some patients unable to bend their fingers and becoming deformed; in some cases, the knee joints are constantly destroyed, making it impossible to walk at all, and they end up in bed.  In the past, rheumatoid arthritis was known as the “undead cancer”, but in the last decade or so, with the continuous progress of medicine, the treatment of rheumatoid arthritis has been enriched, and there are many effective drugs and advanced treatment methods. Most patients can get good control of their disease. Taking early and regular treatment for rheumatoid arthritis is the most important principle to avoid joint destruction and deformity. Preventing damage to the joints and protecting joint function can maximize the quality of life of patients and reduce the risk of disability.
