It is a bright red vaginal discharge containing more blood and a larger amount, usually similar to or slightly more than the usual menstrual volume, sometimes with blood clots (total amount is about 500 to 1000 ml). This is the blood that is shed with the meconium of the uterus, especially the meconium at the placental attachment, and the necrotic meconium and other tissues. Normal malodor has a bloody odor, but no foul smell. Normally, postpartum dew is divided into three stages: 1. Bloody dew: bright red, containing a lot of blood, in large amounts, sometimes with small clots. There is a small amount of fetal membranes and necrotic meconium tissue. Bloody nightmare lasts for 3 to 4 days, the amount of uterine bleeding gradually decreases and the plasma increases, transforming into plasma nightmare. 2. Plasma malignant fluid: light red in color containing a lot of plasma. A small amount of blood, but more necrotic meconium, cervical mucus, uterine exudate, and bacteria. Plasma nightmare lasts for about 10 days, the plasma gradually decreases, the white blood cells increase and turns into white nightmare. 3.White malodex: sticky and white in color. It contains a lot of white blood cells, necrotic tissue meconium, epidermal cells and bacteria. The white nightmare lasts for 3 weeks. Clinically, we can see that the duration of nightmare varies from person to person, depending on the mode of delivery, maternal constitution, postpartum conditioning, etc. The average is about 21 days, with the shortest being 14 days and the longest being up to 2 months. In Chinese medicine, if the bleeding does not stop after three weeks, it is called “incomplete dew”. It is believed that it is mostly related to Qi deficiency and blood stasis. As the ancient saying goes, “Because the blood of the menstrual flow is injured during childbirth, the deficiency is insufficient and cannot be absorbed, or the bad blood is not exhausted, the good blood is difficult to rest, and it goes down together, for a long time.” It is believed that if the malodorous dew is not exhausted, it will injure the Qi and Blood for a long time, causing Qi to be taken off with the blood and deficiency of both Qi and Blood, which will have a great impact on the recovery of the mother’s body and breastfeeding. In terms of treatment, if the color and smell are not abnormal, the treatment can be based on Chinese medicine to identify the severity of qi deficiency and blood stasis, and choose to benefit qi and replenish blood, or activate blood circulation and remove blood stasis, or both, using Chinese medicine for treatment and conditioning, which can often achieve better results. If the mother has fever, pain in the lower abdomen, increased, cloudy, dirty and smelly malodorous dew, it should be considered that it may be related to puerperal infection, in which case anti-infection treatment should be applied.
