Secretory otitis media is a non-purulent inflammatory disease of the middle ear characterized by fluid accumulation in the tympanic cavity and hearing loss. The fluid accumulation in the middle ear can be either plasmacytic leaky fluid or exudate, or mucus. The name of this disease is not uniform, but it is called exudative otitis media, cicatricial otitis media, plasmacytoid otitis media, plasmacytoid-mucus otitis media, and nonsuppurative otitis media. The middle ear accumulation coating is sticky and gelatinous, called gelatinous ear. Causes of secretory otitis media: 1. Eustachian tube obstruction or malfunction: the basis for the occurrence of this disease, such as adenoid hypertrophy, hypertrophic rhinitis, nasopharyngeal tumors, nasopharyngeal filling, etc. 2.Infection: Upper respiratory tract infection, in addition to making the eustachian tube dysfunctional, light infections can also invade the middle ear. Since bacteria and viruses are found in the effusion of the middle ear, it is believed that secretory otitis media is a light or low virulence bacterial infection of the middle ear. 3. Immune response: Because of the high rate of positive bacterial culture in middle ear effusion in recent years and the detection of bacteria-specific antibodies from the effusion, it is suggested that chronic secretory otitis media is a damage to the middle ear mucosa caused by type III metaplasia. 4.
