  I. Discharge instructions
  In order to ensure accurate discharge checkout service and to facilitate you to bring all necessary hospitalization information and discharge medication, we usually handle tomorrow’s discharge, you can go to the first floor of the inpatient department with the “hospitalization deposit slip” from 8:30am to 5:10pm the next day for discharge procedures, and then go to the nurse station of our department to get the discharge medication and discharge If you wish to be discharged on the same day, you must apply to the doctor in charge before 12:00 a.m. on the same day, but you may not be able to take out the medication you are discharged on the same day. It is generally recommended that you apply for tomorrow’s discharge. If you need to make copies of relevant information, please ask the doctor in charge when you apply for discharge. Generally, we choose to lead you to make copies of relevant information with your ID card three days after discharge.
  II. Notes on taking medication
  1.Patients with coronary heart disease after stent implantation generally need to take the following drugs for a long time (refer to the specific discharge with drugs).
  (1) Aspirin enteric soluble tablets, 0.1g, once a day, must be taken for life. If you experience bleeding gums or stomach pain, please consult your cardiovascular physician in the outpatient clinic for continued use.
  (2) Clopidogrel, 50~75mg once daily, must be taken for 1~1.5 years.
  (3) Angiotensin-converting enzyme inhibitor or angiotensin receptor blocker, once daily, for as long as possible.
  (4) β-blockers, once daily, for as long as possible.
  (5) Statin lipid-regulating drugs, to be taken at night, once a day, for as long as possible.
  Note: The time of taking unmarked drugs is usually chosen after breakfast. Please follow the advice of the doctor in charge for special medication.
  2.Patients after radiofrequency ablation: Generally, they should take aspirin enteric soluble tablets (BAY aspirin) 0.1g once a day for at least three months, and if combined with coronary heart disease and hypertension, it is recommended to take it for life.
  3. For patients with permanent pacemakers, oral antibiotics are generally recommended for at least 2 weeks after the removal of stitches.
  Note on rehabilitation.
  1.Pay attention to reasonable diet Patients with coronary heart disease should have low salt and low fat diet. Some patients gain weight when they are hospitalized due to less activity, which is not good for the recovery of the disease. After discharge, patients should reduce the intake of animal fat and cholesterol-containing food (such as fatty meat, animal offal, egg yolk, roe, etc.), advocate a light diet, limit sugar and salt, increase vitamin-rich nuts, vegetable protein (soy products) and fresh vegetables and fruits, eat only seven or eight portions of each meal, work and rest should be regular, quit smoking and limit alcohol.
  2, pay attention to exercise moderate amount of patients after coronary heart disease stenting patients are generally recommended to gradually increase the amount of exercise and labor, general patients can work normally after 3 months (6 months for patients with high age or slow recovery), but the work should not be too strenuous and excessive pressure. After discharge from the hospital, a fitness program can be formulated according to their own situation and moderate activities can be carried out, which is beneficial to the recovery of physical strength and the improvement of blood circulation throughout the body, and is conducive to the establishment of collateral circulation at the site of myocardial infarction. Exercise program can choose walking, aerobics, taijiquan, etc.
  3, pay attention to the observation of the condition of patients with coronary heart disease after discharge from the hospital may occur again heart attack, arrhythmia, cardiac insufficiency and cerebrovascular disease, so you should pay attention to the changes in self-perception. If you have newly appeared or aggravated symptoms such as panic, shortness of breath, chest tightness, chest pain, dizziness, numbness and weakness of the limbs, you should immediately go to the nearest hospital for consultation.
  4, pay attention to the prevention and control of infection Patients with coronary heart disease should always pay attention to climate change, timely increase and decrease clothing, to avoid cold heat stroke. If there is a cold, fever, diarrhea and other diseases should be treated immediately to prevent inducing cardiac insufficiency or arrhythmia.
  5, pay attention to regular review Patients with coronary artery disease should go to the hospital for regular follow-up, while actively treating other diseases such as hypertension, diabetes, dyslipidemia, liver and kidney disease, because these diseases can also trigger or aggravate heart attack.
  6.Be careful not to stop drugs indiscriminately Most of the drugs taken out of the hospital when you are discharged are drugs you must take, such as aspirin and clopidogrel, which are related to the safety of coronary arteries after cardiac interventional surgery, and these drugs need to be taken regularly for at least one year, and after one year aspirin and statins are recommended to be taken for life. As for when to stop using the drugs, a specialist in cardiovascular medicine is needed to decide, and the drugs should never be stopped at will, otherwise many dangerous situations such as subacute thrombosis in the stent may occur. In addition, antihypertensive drugs, hypoglycemic drugs, lipid regulators and other drugs should not be easily discontinued. Otherwise, it may also cause damage to the cardiovascular system or other important organs such as kidney and brain.
  IV. Notes for follow-up consultation.
  1. For patients with coronary heart disease, it is recommended that patients with acute myocardial infarction should come to our hospital for review in six months, and patients with non-acute myocardial infarction should come to our hospital for review in about one year; all patients with coronary heart disease should come to the local hospital for outpatient review every 1~2 months/time to adjust their medication in time.   2. Patients with permanent pacemakers should prevent excessive and strenuous activities of the upper limbs on the side of the pacemaker for 3 months after discharge, avoid going to places with strong magnetic fields, and explain the situation to the security personnel when passing through security checks at airports and customs to prevent adverse effects on the pacemaker. After discharge from the hospital, come to our hospital or a local hospital that has the conditions to adjust the pacemaker every 3 months.
