Cervical cervical spondylosis is a cervical muscle syndrome with limited pain in the head, neck, shoulder and arm and its corresponding pressure points, and degenerative changes such as changes in the physiological curve of the cervical spine and intervertebral instability and mild osteophytes on x-ray. Cervical cervical spondylosis, also known as localized cervical spondylosis, is mostly curable with non-surgical treatment, but is prone to recurrent attacks. Therefore, many authors rarely mention this type in textbooks.
There are ten main treatment measures for cervical cervical spondylosis, and the emphasis is on prevention. To carefully find the cause, improve the living conditions and working environment; to actively carry out functional exercise is the main means of treatment, if you can adhere to, the long-term effect is often unexpected; traction has been widely popular and effective at home and abroad, depending on the person, choose to carry out; acute period around the collar fixed or massage massage short-term effect is excellent, should be identified and treatment, appropriate; closed properly selected;
1, looking for the cause of the disease.
Improve the living conditions and working environment (on the work to cure the disease), strong prevention and removal of causative factors (mend the sheep). Such as avoid high pillow sleep (side lying as 1 shoulder. Supine such as 1 fist, pillow generally 10-12cm high, slightly narrow width, only cushion the neck to maintain cervical curvature.) The neck, shoulders and back should be kept warm;
Long-term ambulation half an hour, to stretch, deep breath; to sit like a bell, stand like a pine. The cervical spine should be kept warm.
2, traction.
Its mechanism and effect so far lack of scientific conclusions, but it is widely popular and effective at home and abroad. It can make the neck brake, maintain a neutral position of the normal neck posture. Its effects are.
(1) Widen the vertebral space, restore the stability of the cervical thrust and its normal physiological function.
(2) Relieving muscle spasm, stripping and loosening adhesions, eliminating inflammation and edema.
(3) To distract the embedded joint capsule in order to promote the rectification of vertebral body slippage and hook vertebral joint misalignment.
(4) Enlarging the intervertebral foramen and the intervertebral space, and distracting the folded or ossified ligaments to relieve the compressed nerve roots and spinal cord, improve their blood supply, and help restore the function of the neural tissues. x-ray study proves that after traction, the intervertebral space can be widened by 2.5-5 mm, and the longitudinal diameter of the cervical spinal canal can be extended by more than 10 mm.
(5) Traction opens the tortuous vertebral artery, restores the proportional relationship between the length of the cervical spine and the vertebral artery, keeps the vertebral artery open, maintains normal cerebral blood supply, and restores normal brain tissue function.
In short, traction can adjust and restore the internal and external balance of the cervical spine, and then restore the normal physiological function of the cervical spine. It has been studied that after cervical flexion of 15° and 6 kg of traction, the pressure of cervical intervertebral disc decreased by 50% and the overall displacement of cervical spine increased by 50% on average. Therefore, traction can make pain symptoms significantly relieved or gradually disappear.
In the state of traction, the pressure of weight on the disc can be removed; the compression of the disc and its surroundings caused by the muscle contraction and the tension of the ligaments can be reduced. So that the disc and its surroundings in the state of non-load, conducive to the intervertebral disc and its surrounding nutrient supply, so that the injury of the annulus fibrosus and its surroundings can be repaired; conducive to the intervertebral disc and its surrounding venous reflux, eliminate edema and promote the inflammation to subside.
3.Massage and tui-na.
Massage therapy is one of the most common and short-term extremely effective methods in order to relieve the symptoms of cervical cervical spondylosis in the acute stage. Massage therapy for cervical spondylosis has the effect of relaxing tendons and channels, activating blood circulation and dispersing blood stasis, relieving swelling and pain, slipping and repairing joints, but the long-term efficacy of chronic cervical cervical spondylosis is still controversial.
4. Peripheral collar.
Cervical spondylosis in the acute stage of fixation is conducive to the decreasing edema of tissues, reducing the trauma response and eliminating symptoms quickly. However, the time should not be too long, easy to cause muscle atrophy.
5.Closure.
For acute limited cervical, back and shoulder pain, sometimes there is the effect of “needle to remove the disease”, so it is widely used. However, the long-term efficacy of chronic cervical cervical spondylosis lacks strong data support.
6.Physiotherapy.
Physiotherapy is widely used in clinical practice and has its own unique medical value as an adjunct to the treatment of cervical spondylosis. Cervical spondylosis often appears as stubborn neck and shoulder pain, and electrotherapy and heat therapy have good pain relief effects.
When cervical spine osteophytes compress nerve roots and spinal cord, it can cause inflammatory reaction. The application of ultrasound, infrared, electrotherapy, and heat therapy can produce the effect of promoting the inflammation to subside and absorbing edema. Inflammatory reactions over time can cause tissue adhesions, and post-surgical patients often have a large amount of scarring. Physiotherapy has the effect of loosening the adhesions and softening the scar.
7.Western and Western medicine internal and external treatment method.
It is an important means of “supporting the righteous and eliminating the evil”. Non-steroidal drugs are widely used, such as aspirin, paracetamol, brevifene, Xilabao, etc.; and Zu Shi Ma (dispel wind, cold and dampness), Duyi Wei soft capsule (treating wind first and blood, and the wind will be extinguished when the blood is moving), etc.
8, functional exercise.
This is a lasting but extremely effective hope project, if you can adhere to, the long-term effect is often surprising. The purpose and role of functional exercises for the cervical muscles in cervical cervical spondylosis are mainly twofold.
① strengthen the neck muscles, enhance their ability to tolerate fatigue and improve the stability of the cervical spine, thus consolidating the treatment effect and preventing recurrent attacks ;
② Through relaxing exercises in all directions of the neck, the blood circulation in the cervical spine area is activated to eliminate stasis and edema, while stretching the ligaments of the neck, relaxing the spastic muscles and regulating the stress of the cervical spine, so as to reduce the symptoms.
In the acute phase of cervical cervical disease, it is advisable to rest locally and it is not advisable to increase exercise stimulation; in the remission of symptoms, exercises based on cervical muscle isometric exercises can be carried out; when the symptoms basically disappear or are in a chronic state, you can start
(1) cervical spine mobility training, (2) cervical spine function training
(2) functional exercise of cervical spine
(3) strengthening cervical muscle exercise to promote the further elimination of symptoms and consolidate the therapeutic effect. Cervical spine mobility training, functional exercise and cervical muscle strengthening exercise must be noted: if there are more obvious or progressive symptoms of nerve root and spinal cord compression, they should be stopped at any time. And promptly conduct further examination to exclude cervical spondylosis other than cervical-type cervical spondylosis.
(1) Mobility training for cervical cervical spondylosis: In general, it is the movement of the cervical spine in six directions (without resistance).
The more common and easy-to-use mobility training for cervical cervical spondylosis is represented by the “rice” exercise, which means that the head is imagined as a “pen” and a “rice” is drawn in the air through the rotation of the neck. The “rice” exercise involves imagining the head as a “pen” and making a “rice” stroke in the air by turning the neck. The “rice” exercise contains cervical flexion and extension, lateral flexion and rotation and other movements, and contains several different movements of the composite action. At present, many people like to do the “rice” exercise, and believe that the “rice” exercise can cure cervical cervical spondylosis.
Step 1: Prepare for the posture: stand with your feet shoulder-width apart and cross your arms.
Step 2: Left and right lateral flexion exercise: left side of the head flexion – square – right side flexion – square. Repeat 10 times.
Step 3: Left and right lateral rotation exercise: turn your head on the left side – pose – turn on the right side – pose. Repeat 10 times.
Step 4: Nodding and tilting exercise: head tilting forward and nodding – posing – tilting back – posing. Repeat 10 times.
Step 5: Rotation movement: head from the front – left – back – right – front direction to do counterclockwise rotation for a week, and then from the front – right – back – left -The direction of the front to do clockwise rotation for a week. Repeat each 5 times.
Note that the head movement should be slow and return to the middle position each time the head is moved.
(2) Functional exercise for cervical cervical spondylosis: In summary, it is the activity of the cervical spine in six directions (resistance).
The first section of shoulder contraction exercise: relax the neck and keep the head in the middle position. Slowly lift both shoulders upward as far as possible, maintain for 5 seconds and then relax, repeat 15-20 times.
Section 2: Place your hands on your forehead and slowly press your head forward, keep your hands blocking the force of the head press, then place your hands behind your head and press your head backward, the rest of the steps are the same as before.
The third section and the item to compete with the force: cross the fingers of both hands on the back of the neck, hands forward force at the same time the head of the neck backward force to do resistance movement, maintain 5 seconds and then slowly relax, repeat 10 times.
Fourth section left (right) hand on the left (right) side of the face, the head slowly pressed to the left (right) side, the hand continues to block the head pressure to the force. Maintain for 5 seconds and then slowly relax and repeat 10 times. Rotate the left (right) muscle group exercise: slowly rotate the head to the left (right) side to apply pressure, the left (right) hand to block the force of the head rotation pressure to keep the head in the middle position. Maintain for 5 seconds and then slowly relax, repeat 10 times.
Fifth section of the neck self-traction: head in the middle position, one hand to hold the pillow, the other hand to hold the lower jaw, hands at the same time slowly upward force traction, maintain 6 seconds, slowly relax, repeat 15-20 times.
Section 6 massage to relax the neck: five fingers of both hands press the scalp, massage from front to back, make the scalp feel sore and swollen, repeat 15-20 times.
(3) Strengthen the cervical muscle exercise method: In general, it is to strengthen the cervical extension muscle strength and restore the cervical curvature.
The authors noted that the strengthening cervical muscle exercise method borrowed from “cervicogenic vertigo” is beneficial to the treatment of cervical cervical spondylosis.
The method of exercise is: sitting or prone position, both upper limbs straight and placed behind, both hands crossed (or not crossed if it is difficult to cross), both arms try to extend backward, and at the same time lift the head as hard as possible (it is advisable to be slow), tense the muscles of the back of the neck and the muscles between the shoulder blades as much as possible, continue for 10 seconds and then stop and return to normal position, try to relax the tensed muscles, rest for 10 seconds and then do the above exercise again, repeatedly. Exercise to feel fatigue or slight sweating can be stopped, should not be an excessive exercise, 3-5 times a day exercise, often this exercise will effectively promote the recovery of the cervical physiological curvature and strengthen the relevant muscle strength, enhance the stability of the cervical spine.
9.Warmth.
Avoid wind/cold/damp (evil) invasion. Such as air conditioning, electric fan, cold bath, window wind (at night/in the car/sleep), humidity, cold, seasonal and seasonal changes, etc.
