What does a low free PSA vs. total PSA indicate?

A low free PSA and total PSA indicates that it is normal, and is only clinically significant if it is elevated.
PSA refers to prostate-specific antigen, a protein secreted by the epithelial cells of the prostate and periurethral tissues, which is increased in prostate cancer, benign prostatic hyperplasia, prostate inflammation, and rectal examination.
Clinically, PSA test is mainly used for prostate cancer screening.
The concentration of total PSA is normally below 4.0 μg/L, above 4.0 μg/L suggests the possibility of prostate cancer, and when it exceeds 10.0 μg/L, the possibility of prostate cancer is very high and prostate aspiration biopsy is required.
The free PSA percentage is only meaningful if the total PSA is between 4.0 and 10.0 μg/L. The lower the free PSA/total PSA percentage, the higher the possibility of prostate cancer.
