Raltitrexed can be administered either intravenously with preoperative neoadjuvant chemotherapy or intraperitoneally during surgery. Raltitrexed is primarily a thymidylate synthase inhibitor, an antimetabolite folate analog. It is indicated for the treatment of patients with advanced colorectal cancer for whom 5-Fu/calcium folinate is not indicated. The drug can be used prior to surgery and is indicated for some patients with advanced colorectal cancer who are unable to undergo combination chemotherapy. Continue to use the drug after administration if no toxicity occurs. Raltitrexed can also be administered intraoperatively by intraperitoneal injection. Contraindications to this drug are severe hepatic and renal insufficiency; pregnancy, and lactating women. Adverse reactions include nausea and vomiting, diarrhea, leukopenia, elevated aminotransferases, abnormalities in heart rhythm and cardiac function, rash, and fever.
