Can 85% stenosis of the first diagonal branch be treated conservatively?

For patients with 85% stenosis of the first diagonal branch, if the patient has obvious evidence of ischemia (e.g., retrosternal pressure pain or a feeling of suffocation) or if the monitored flow reserve fraction is ≤0.8, surgical intervention at the lesion site is recommended, and conservative treatment with medications has limited effect.
Diagonal branch is the branch of the anterior descending branch of the heart to the free wall of the left ventricle. When the cardiac blood vessels are narrowed to a certain condition, the arterial blood supply can be drastically reduced or interrupted, so that the corresponding myocardium is severely and persistently acutely ischemic leading to myocardial necrosis. In severe cases, malignant arrhythmia, shock, heart failure and other threats to the patient’s life can occur suddenly.
It is recommended that according to the local hospital conditions and the actual situation of the patient’s condition, on the basis of drug treatment, improve the examination by a specialist physician to assess whether surgical intervention is needed.
