Meniscal injuries are often evaluated and graded using MRI. Generally, meniscal injuries are categorized as grades 0 to 3 on MRI. Grade 0: The meniscus has normal morphology, smooth surface, and uniform low-signal shadow inside. Grade 1: is a meniscus with normal morphology and smooth surface, but with a small flaky or punctate high signal in the center. Grade 2: The meniscus shows transverse, oblique, or longitudinal strips of high signal on MRI, usually in contact with the joint capsule, but not yet reaching the edge of the meniscus. Grade 3: The meniscal injury shows a multiblock presentation with displacement into the joint cavity and complete or partial loss of structural integrity.
