When the levels of TT4, FT4, TT3 and FT3 are reduced and TSH is elevated, TG antibodies and TPO antibodies are elevated, suggesting Hashimoto’s thyroiditis. The 7-item thyroid function test includes TSH (thyroid stimulating hormone), total T3 (total triiodothyronine), total T4 (total thyroxine), free T3 (free triiodothyronine), free T4 (free thyroxine), and TPO antibodies (thyroid peroxidase antibodies), TG antibodies (thyroglobulin antibodies). When TT4, FT4, TT3, and FT3 levels are elevated and TSH is decreased, it suggests primary hyperthyroidism; when TT4, FT4, TT3, and FT3 are elevated and TSH is elevated, it suggests possible central hyperthyroidism. When TT4, FT4, TT3, and FT3 levels decrease and TSH increases, TG and TPO antibodies increase, suggesting hypothyroidism in Hashimoto’s thyroiditis; and TT4, FT4, TT3, and FT3 decrease and TSH decrease, suggesting possible central hypothyroidism. When only TSH is elevated, but others are normal, subclinical hypothyroidism is suggested.
