Severe splenomegaly, also known as giant spleen, has a lower border that extends below the level of the umbilicus. It can be seen in Gaucher’s disease, chronic granulocytic leukaemia, chronic malaria, advanced schistosomiasis, true erythrocytosis, thalassaemia and myelofibrosis.
