[Abstract] Objective To determine urinary transferrin (TRF), type IV collagen (IV-C) and N-acetyl-beta-D-glucosaminidase (NAG) in the early diagnosis of diabetic nephropathy in type 2 diabetic patients. Clinical significance. Methods: Urine TRF, IV-C, NAG, microalbumin excretion rate (UAER), urinary creatinine (Cr) and other related biochemical parameters were measured in 60 type 2 diabetic patients and 20 normal subjects in 24-hour urine. The urinary TRF and IV-C, which reflect the impairment of glomerular function, and the urinary NAG, which reflects the impairment of tubular function, were significantly higher in the diabetic urinary microalbumin normal group and the microalbuminuria group, and showed a significant positive correlation with UAER. Conclusion: Urinary TRF, IV-C and NAG are highly sensitive in early prediction and screening of diabetic nephropathy, and tubular impairment exists at the same time as glomerular damage in early diabetic nephropathy. Abstract】 Objective To evaluate the clinical significance of detection of urinary Transferrin(TRF), type IV collagen(IV-C), N-acetyl-beta-D- glucosaminidase(IV-C), and N-beta-D-glucosaminidase(IV-C). Diabetic nephropathy (DN) is a common and intractable microvascular complication of diabetes mellitus (DM), and is one of the major causes of death in DM. We have examined urinary transferrin (TRF), type IV collagen (IV-C) and N-acetyl-beta-D-glucosaminidase (NAG), which reflect the damage of glomerular function. The clinical value of these tests for the early diagnosis of DN was investigated.
1 Subjects and methods
1.1 Subjects
1.1.1 Type 2 diabetes mellitus group Sixty patients, 34 males and 26 females, were diagnosed with diabetes mellitus by the endocrinology department of our hospital. The age was 53.54±11.14 years, and the disease duration was 0.1-16 (5.77±4.42) years. All met: (1) confirmed diagnosis of type 2 diabetes mellitus according to World Health Organization criteria (1999); (2) no serious liver or kidney disease, non-diabetic renal disorders, ketoacidosis, hypoproteinemia and urinary tract infection, hyperthyroidism, heart failure, or recent history of cerebrovascular accident; (3) no recent use of nephrotoxic drugs, calcium antagonists and angiotensin-converting enzyme inhibitors.
