I. Definition
Proliferative arthritis of the knee is a common and frequent disease in middle-aged and elderly people, also known as osteoarthritis of the knee, and belongs to the category of “paralysis” in Chinese medicine.
II. Clinical symptoms and signs
1. Patients have pain in the knee joint, with varying degrees of limitation in flexion and extension; it is usually worse at night, aggravated by exertion, with difficulty in walking up and down slopes and stairs.
2. Some patients have a history of knee swelling or effusion and have difficulty walking.
3. some patients have a strangulation sensation in the knee joint, and examination reveals one or several fixed pressure points around the patella or on either side of the extensor fascia.
4. some patients may have palpable hyperplastic hypertrophied knee bursa.
5. Some patients have mild atrophy of the quadriceps muscle of the affected limb.
Confirmation of diagnosis
The 1995 American College of Rheumatology diagnostic requirements are 5.
1. Knee pain on most days of the month.
2. X-rays showing bone formation at the bony end of the knee.
3, Age greater than or equal to 40 years.
4, morning stiffness of the knee joint less than or equal to 30 minutes.
5, popping when the joint is moved.
The presence of 1, 2 or 1, 2, 4, 5, can diagnose osteoarthritis of the knee joint.
IV. Treatment
Clinical treatment of this disease is mainly conservative and surgical
1, conservative therapy.
(1) drugs.
(2) external application.
(3) physical therapy.
(4) acupuncture.
(5) closure.
(6) sodium vitrate (also known as hyaluronic acid) injection into the joint cavity.
(7) small needle knife relaxation.
(8) massage and tendon manipulation for joint benefit and pain relief techniques.
(9) ozone therapy and other comprehensive therapies.
All of the above therapies can achieve satisfactory clinical results.
2.Surgical treatment.
(1) Knee arthroscopy.
(2) Total knee replacement and other therapies.
V. Functional training of the knee joint
1, quadriceps incompetence exercise: patients can lie down or take a seat, lower limbs straight, active quadriceps contraction movement, to the degree of quadriceps soreness (note: slow movement).
2, backward pedaling bicycle: patients lying flat, lower limbs bending knees and hips, pedaling action, as slow as possible, three groups per day, 20-30 times a day, to the abdomen slightly hot and swollen as degree (or backward pedaling exercises when riding a bicycle, electric car).
3, bedside water sports: the mouth can sit on the edge of the bed or chair, do alternate calf straightening movement (to slow), three groups per day, 20-50 times per group, to the front of the calf with soreness and swelling as degree.
Precautions.
1, rest and warmth, to prevent cold.
2, knee exercises are strictly prohibited when standing, walking with weight, long walks, climbing and other sports.
3, to carry out knee exercises strictly in a weightless state, especially to strengthen the quadriceps exercise to correct the patellar displacement to restore the normal position of the patella and restore the force balance disorder of the patella.
