Severe tuberculosis has different CT manifestations according to the different disease subtypes such as primary tuberculosis, hematogenous disseminated tuberculosis, infiltrative tuberculosis and cavitary tuberculosis. 1. Primary tuberculosis: severe foci, CT may show marginal mottled and nodular foci in the posterior segment of the upper lobe of the lungs and the dorsal segment of the lower lobe of the lungs, which may be accompanied by enlarged hilar and mediastinal lymph nodes and manifestations of fibrosis or calcification. 2. Hematogenous tuberculosis: most of them will have corn-like shadows with uniform distribution of size and density from the lung tip to the lung base, which may be accompanied by exudation around the foci; meanwhile, in serious cases, the corn-like shadows may be enlarged, fused, and blurred at the edges in the later stage of the disease. 3. Infiltrative tuberculosis: manifested as high density in the center and thin and blurred patchy, small or cloudy shadows at the edges. Most of them are multiple, which can be in one lung segment or lobe, or distributed in one side or both sides of the lung field; and caseous lysis can be seen in the lesion. 4. Cavitary tuberculosis: low-density translucent areas may be formed or cavities may appear, showing translucent areas of different sizes and shapes. Synchronized speckled and nodular shadows can be seen around the cavities, which are called “satellite foci”.
