With the modern medical research on psoriasis and the influence of microcirculatory disorders in the disease, as well as the deepening understanding of Chinese medicine on the etiology and pathogenesis of the disease, the most representative of them are: the three TCM subtypes of blood heat, blood stasis and blood deficiency, which mainly adopt the method of blood management, that is, clearing heat and cooling blood, nourishing blood and activating blood circulation, nourishing blood and moistening dryness as the basic principles of treating psoriasis. A large number of clinical practices have confirmed that the Blood Management Method plays a pivotal role in the treatment of psoriasis, of which activating blood circulation and removing blood stasis is the core of the Blood Management Method in the treatment of psoriasis. Chinese medicine practitioners of all ages have unique insights into the application and exposition of the Blood Management Method in various diseases, which also has guiding significance for the treatment of psoriasis to this day. Emphasis is placed on clearing heat and cooling the blood. Blood heat can be caused by a variety of factors such as external factors (wind, cold, dampness, dryness and fire), internal injuries caused by the seven emotions and dietary imbalance. Chinese medicine believes that various internal and external pathogenic factors can lead to Qi stagnation, heat in the blood, and heat in the skin. It is equivalent to the modern medical belief that factors such as respiratory infections, mental and emotional changes and diet can induce psoriasis or aggravate the disease. The manifestation of psoriasis, whether it is common type, joint type, pustular type or erythrodermic type, or in the progressive stage, can show symptoms of blood heat. The skin lesions are {red and the rash is increasing, accompanied by heart trouble, dry mouth and thirst, dry stool and short red urine, with a bright red or vivid red tongue and slippery pulse. Treatment is based on the identification of the symptoms and the use of heat-clearing and blood-cooling drugs, such as water hyacinth, salvia, tannin, red peony, raw earth, comfrey, etc. In addition to nourishing blood and moistening dryness, Chinese medicine believes that blood deficiency can be caused by the body’s congenital endowment deficiency, deficiency of blood in the campuses after prolonged illness, wind and dryness, and loss of skin nourishment. It is equivalent to the modern medical belief that the genetic family history of psoriasis patients, the disorder of the body’s immune function, and the long history of the disease are related. Clinically, psoriasis is prolonged, stubborn and difficult to heal, with weak constitution, pale erythematous spots, silvery white flakes that are loose and abundant and easy to fall off, and accompanied by itching of different degrees. Treatment is often used to nourish blood to remove wind and nourish Yin and moisten dryness, such as angelica, rehmannia, chicken blood vine, Chuanxiong, sage, maitong, etc., often with good results. Modern medical research has found that patients with psoriasis have varying degrees of blood rheology changes, which are characterized by “thick, sticky and coagulation”, irregularly curved capillaries in the nail folds or dermis, short blood vessel wall loops, and dotted bleeding, which is equivalent to the stasis of blood in Chinese medicine. This is consistent with the thickening of skin lesions, nail fault, purple and dark tongue or stasis of blood symptoms seen in psoriasis patients in Chinese medicine. Blood-stasis activators can activate blood circulation, reduce blood viscosity and improve microcirculation. Chinese herbs such as Salvia miltiorrhiza, peach kernel, safflower, trigonella, curcuma longa and red peony are commonly used. At the same time, activating blood circulation and resolving blood stasis is used throughout the treatment of psoriasis. In the Qing Dynasty, Wang Qingren and other traditional Chinese medicine practitioners had the discussion of “blood heat and heat poison decoction into stasis”, “long-standing disease has stasis” and “invigorating blood to nourish blood and remove wind”. Therefore, in the TCM treatment of psoriasis, regardless of the evidence of blood heat, blood stasis or blood deficiency, there is a need for blood-activating and stasis-removing drugs. When using the method of activating blood to eliminate stasis, it is also necessary to add rehmannia, shouwu and other yin and blood tonifying drugs as appropriate when the disease has been deficient for a long time. In the case of old age and physical weakness, we can add Radix et Rhizoma Atractylodis Macrocephalae and Rhizoma Atractylodis Macrocephalae to nourish Qi and strengthen the immunity of the body. Some blood-activating drugs may have hypotensive effects, so patients with low blood pressure should be used with caution. In a word, the method of blood circulation is used throughout the treatment of psoriasis. In clinical practice, the three methods are used for different causes, different conditions and different stages, each with its own focus, and with the addition of heat-clearing and detoxifying herbs as appropriate, which can often receive twice the effect with half the effort.
