I once encountered such a case, a girl who studied in a key middle school, with good character and good academic performance, if there is no accident, should not be a problem to enter a major national university, but in her senior year of high school; such symptoms appeared: frequent epigastric pain, unbearable pain during attacks, accompanied by nausea and vomiting, sweating profusely, because it is unbearable, even crying and shouting, went to a number of large hospitals, internal medicine, gastroenterology, related to all the tests, no abnormalities were found. All the examinations did not reveal any abnormality, and the internal medicine department had given pain relief and medicine to relieve gastrointestinal spasm, which was slightly improved after treatment, but the attacks were repeated, and there was a pattern that the above-mentioned symptoms were aggravated once the school was in session. Later, he was prompted to see a psychiatric clinic. Due to the patient’s chronic poor eating with vomiting, he was thin, emaciated and weak at this time. Considering his physical condition, he was given an antidepressant with less side effects, and after more than a month of treatment, his condition improved significantly, and the interval between attacks and the degree of pain were significantly reduced, but he still had attacks from time to time. Therefore, the patient was switched to an antidepressant with better efficacy in the treatment of somatization disorder. After the switch, the above symptoms did not recur and the patient successfully passed the college entrance examination. In layman’s terms, somatization disorder is feeling physically uncomfortable, but after various physical and chemical examinations, all kinds of objective abnormal changes are not found, or minor abnormal changes are found that cannot explain such a serious clinical manifestation. These patients are often repeatedly examined in major general hospitals, and no organic changes are found from the perspective of internal medicine or neurology, but the patient feels very uncomfortable, and an experienced internist will recommend a psychiatric consultation. The pathogenesis of this disease is not very clear, generally there are certain psychosocial factors, the main manifestations, gastrointestinal, cardiovascular and other symptoms, symptomatic treatment in internal medicine has little effect, but the use of antidepressants can achieve significant results, in recent years this disease seems to have an increasing trend, according to conservative estimates, in internal medicine, neurology patients may be no less than 20% of similar diseases. For the layman, you only need to remember one thing: if you feel uncomfortable, but no abnormalities are found after various tests, go to the psychiatric department.
