How effective is immunotherapy for bile duct cancer

Immunotherapy treatment can be used as an adjuvant treatment for cholangiocarcinoma and can improve the prognosis for some patients. The prognosis of cholangiocarcinoma patients is often poor, and surgery is the main treatment option, but for middle and late stage patients, they often lose the chance of surgery or need to combine radiotherapy, immunotherapy and targeted therapy. Immunotherapy for cholangiocarcinoma mainly includes the following aspects: 1. immune checkpoint inhibitors: some studies claim that in unresectable or recurrent patients with highly microsatellite instability or absence of cell mismatch repair mechanism, treatment with immune checkpoint inhibitors pabolizumab, karelizumab or nabumab may benefit patients, but the current clinical application is relatively small. 2. Vaccine: There is a study using autologous dendritic cells loaded with MUC1 as an adjuvant therapy for patients with pancreatic and biliary tract tumors, and 4 out of 12 patients survived for more than 4 years. However, it is currently in the research stage. 3 Passaged cell therapy: a clinical trial enrolled 36 postoperative patients with intrahepatic cholangiocellular carcinoma who were inoculated with autologous tumor lysate-pulsed dendritic cells in combination with ex vivo activated T cells for at least 3 times; it significantly improved the overall survival time of the patients (31.9 months Vs 17.4 months) compared with surgery alone. However, it is not yet approved for cholangiocarcinoma treatment.
