The dangers of incarcerated hernias that you must not ignore

Special pediatric hernia – incarcerated inguinal hernia. Abdominal organs enter the hernia sac and do not reset on their own but stay in the hernia sac. This is a common complication of pediatric inguinal hernia. If not properly treated, strangulated bowel obstruction can occur with serious consequences. Clinical manifestations When an inguinal hernia becomes incarcerated, a painful mass appears in the groin or scrotum. The child cries and is restless, and nausea and vomiting gradually appear later. If not treated in time, the symptoms of intestinal obstruction gradually aggravate, abdominal distension is obvious, and vomiting is intestinal contents. After clamping closure mostly stop defecation. If there are bloody stools and symptoms of poisoning at the same time, it is mostly suggestive of intestinal necrosis. Examination reveals a bulging mass in the groin or scrotum, which is hard, with little push and obvious tenderness. In more advanced cases, the scrotal skin is congested and red, which does not necessarily indicate intestinal necrosis within the hernia, but it is more difficult to distinguish it from scrotal inflammation caused by intestinal necrosis. Treatment Pediatric incarcerated inguinal hernia should be treated urgently. 1, manipulation reset: due to pediatric anatomical and physiological characteristics, hernia content from incarceration to necrosis of the process is relatively slow; hernia incarceration, the hernia sac around the tissue edema, anatomical relationship is not clear, the wall of the hernia sac of the pediatric was thin, edema is more likely to be torn, all of these increase the difficulty of emergency surgery or produce some unexpected complications. Therefore, for the duration of the disease in 12 hours or so of pediatric incarcerated hernia, generally do not rush to surgery, can first try to manipulate the reset. If the repositioning is successful, wait 24-48 hours for the edema to subside before surgery. Manipulation can make the hernia disappear immediately, there are advantages, but there can be serious consequences, the following cases should be contraindicated: (1) incarcerated time has been more than 12 hours; (2) has been tried to manipulate the failure of the restoration; (3) girls incarcerated hernia contents are often ovaries or fallopian tubes, most of them are not easy to reset; (4) newborns are unable to estimate the time of incarceration of the hernia; (5) the systemic condition of the poor, or there has been a blood in the stools, such as the stricture of the signs of the person. 2, surgical treatment: Where manipulation failed to reset or should not be manipulated to reset, should be emergency surgery. Incarcerated inguinal hernia without intestinal necrosis, the prognosis is good. In the late stage of poor general condition, especially neonates, although actively treated, can still produce more serious consequences.
