Complete right bundle branch block is associated with varying degrees of risk, usually in the form of organic heart disease. In patients with complete right bundle branch block, the progression of the heart disease is accompanied by more severe left bundle branch block, which can lead to right heart failure, left heart failure, or even total heart failure. Complete AV block can also cause malignant arrhythmias, which can lead to sudden cardiac death. Patients may experience vertigo, syncope, or even transient loss of consciousness due to low ventricular beat frequency and inadequate cerebrovascular blood supply. If a complete right bundle branch block is not fitted with a pacemaker and the block is not corrected, causing severe bradycardia, the patient can suffer cardiogenic shock or even death.
