Chinese medicine treatment of diabetic diarrhea

  With the increase in the incidence of diabetes, diabetic diarrhea is also gradually increasing. In a survey of 136 diabetic patients in outpatient clinics, 22% had diarrhea for 2 to 3 days per year; among 387 diabetic patients we counted, 15.8% had diarrhea of varying degrees; diabetic neuropathy patients with diarrhea were also reported to be 16.7%.  Diabetic diarrhea generally occurs in patients who have had diabetes for a long time. Its clinical manifestations are alternating diarrhea with several or tens of lines of diarrhea per day, or even, no or mild abdominal pain, with negative stool routine and bacterial culture. Most patients have intermittent diarrhea, and only a few have persistent diarrhea. The diarrhea mostly occurs after eating or at night, and is often associated with mood swings, cold and greasy food, cold attack on the spleen and stomach, and unsatisfactory blood sugar control. Most patients with diarrhea are accompanied by diabetic neuropathy, but the diagnosis can only be made if all causes of diarrhea are excluded.  Diabetic diarrhea is commonly seen in patients with unsatisfactory glycemic control and mostly accompanied by neuropathy; it is also seen in a few patients with satisfactory glycemic control without neuropathy. Most scholars believe that this disease may be related to gastrointestinal vegetative neuropathy, small mesenteric artery lesions, and electrolyte disorders, reduced resistance to secondary infections, and abnormal insulin and glucagon. The most important of these is autonomic (vegetative) neuropathy.  According to Chinese medicine, this disease is caused by diabetes mellitus for a long time, damage to the spleen qi, coupled with eating cold and greasy food, mood swings and cold attack, which seriously injures the spleen, resulting in weakness or coldness of the spleen qi, loss of spleen health, gathering and dampness, stasis and heat, and even injury to the spleen and kidney yang and diarrhea.  Treatment of diabetic diarrhea, if given to astringent antidiarrheal drugs easily lead to constipation, constipation after using laxative drugs and easy to cause diarrhea without and, so the clinical treatment is quite difficult. The side effects of taking western medicine colistin such as drowsiness, head nausea and loss of appetite are easy to occur; the efficacy of using easy to stop and compound benzodiazepine is also unsatisfactory.  According to our statistics, about 90% of diabetic diarrhea patients are suffering from spleen qi weakness or spleen-kidney yang deficiency. Therefore, the main composition is to strengthen the spleen, tonify the qi, and relieve dampness to stop diarrhea. Basic formula: Raw Astragalus, Radix Codonopsis, Ling, Atractylodes Macrocephala, Fried Yam, Chen Pi, Sheng Ren, and Roasted Licorice. If wet, add Cang Zhu, Huo Xiang, Pei Lan; if hot, remove raw astragalus and dang ginseng, add Amaranthus, Bupleurum, Huang Lian; if abdominal pain, add white peony; if bloated, add Hou Pu, Mu Xiang; if Yang deficiency, add Fulong liver, dry ginger, Zi; if swollen, add Ze Xie, Che Qian Zi, Slippery Rock, etc. After treatment received more satisfactory results, most of the 2 to 3 doses of medicine, the number of stools significantly reduced, 1 to 2 weeks stool turned to normal. With said not seen after the medicine constipation, diarrhea episodes and the number of times also significantly reduced. The sympathetic diabetic diarrhea, the pathogenesis of the disease is based on the weakness of spleen qi, diarrhea for its symptoms. The use of bitter-cold, heat-clearing and dampness-drying flavors and scurrying qi products when dealing with concurrent evidence are prone to injury to the spleen, so do not overdo it or use more, and stop in the disease.
