Antiviral therapy for liver cancer

In the past two days, probably due to the impact of the epidemic, the online platform consulting patients more than usual. After consecutively receiving several patients consulting on antiviral treatment, I thought that some patients may have gone to the hospital for regular consultation, but the usual review and follow-up were not satisfactory, and over time, many of the problems that the doctor had told them about were thrown out of the window! Consider this and give a little guidance in conjunction with the medication guidelines. I am a hepatobiliary surgeon with a focus on online counseling for patients with malignancies of the hepatobiliary system. Prior to the 2019 version of the hepatitis B antiviral therapy dosing guidelines, the use of antiviral medications were predominantly for medical patients, and often the doctor told you a few things: whether HbsAg(+), HBV-DNA was greater than 105, whether liver function transaminases were more than twice normal, and whether age was greater than 30 years. There may also be different things that different doctors say about this, and as some patients have said, seeing different doctors ends up being confusing when they don’t know if they should be on medication or not. After the 2019 version came out, the reference questions are simpler, and also the important few guidelines I want to talk to you about:1. As long as there is hepatitis B HBV-DNA positivity (higher than the reference value) and transaminase abnormality, antiviral treatment is needed.2. For people with normal liver function, if there is a family history of cirrhosis or liver cancer, they are in the high-risk group, and if they are older than 30 years of age and have a positive HBV-DNA. Even if the transaminases are normal, active antiviral therapy is recommended.3. For patients with HBV-related hepatocellular carcinoma and patients with malignant tumors of the biliary system, antiviral therapy is recommended as long as HbsAg(+) is present. It should also be emphasized that in the course of treatment, attention must be paid to review and follow-up. According to the results of liver and kidney function and HBV-DNA review, some patients need to change the therapeutic drugs. The epidemic in Shanghai has fluctuated in the past two days, and I’ve spent more time at home than before, so I’m writing something in the hope that it will help you! I hope the epidemic will end sooner rather than later, and I wish our readers good health and all the best.
