Menorrhagia is a condition in which the menstrual cycle, period and volume of menstruation become seriously abnormal, and it means that the menstrual blood is not stopping or dripping out of time. It is one of the common, frequent, difficult and urgent clinical symptoms in gynecology. Professor Liu Ruifen of Shandong University of Traditional Chinese Medicine has been teaching and working clinically in gynecology for nearly 40 years, and has created a general formula to stop bleeding for the bleeding period of collapsing leakage, which has received satisfactory clinical effects. The author had the honor to study with her teacher and benefited greatly. In “Nei Jing Su Wen Yin Yang Bu Bu Lun”, it is said that “Yin deficiency and Yang struggle is called “collapse”, which is the first name of “collapse”. The first mention of “leakage” is found in the “Jin Kui Yao – Women’s Diseases – Pulse Evidence and Treatment” of the Han Dynasty: “A woman who has Y disease and has not had menstruation for three months and has more than one leakage”. 1, etiology and pathogenesis The “Gynecological Jade Rule” gives a comprehensive overview of the disease: “The source of the disease has six ends, one by fire and heat, two by deficiency cold, three by labor injury, four by gas trapping, five by blood stasis, and six by weakness”. Most scholars now believe that the disease has multiple causes and complex pathogenesis, not a single internal organ, often as a result of each other, qi and blood with the disease, multiple organs are involved. According to Liu, the core pathogenesis of the disease is “injury to the flushing and the inability to control the menstrual blood”. Although the causes of injury to the flushing, there are different deficiency, actual, cold, heat, but “stasis, heat, deficiency” intertwined, each suffering from each other as the disease, there are many. 1.1 “Stasis” obstructs the ramifications and blood does not return to the menstrual flow. Depression, liver depression and qi stagnation, qi stagnation and blood stasis, stagnant blood or after menstruation and delivery, when the remaining blood is not yet exhausted, feeling external evil or intercourse with yin and yang, cause stasis to obstruct the ramifications and blood does not return to the menstrual flow, resulting in leakage. As stated in the Treatise on the Origin of Diseases: “There is stasis of blood inside, therefore, the bleeding stops from time to time, and there is constant dripping”. 1.2 “Heat” disturbs the ramifications and forces the blood to move delicately. If the body is full of Yang, or if it eats spicy and Yang-boosting products, or if it is full of fire and heat, heat disturbs the ramifications and forces the blood to move delicately, thus causing the leakage. As stated in Fu Qingzheng’s “Women’s Medicine”, “The blood boils when the rushing pulse is too hot. The blood collapse is a disease, which is due to the too much heat of the rushing veins. 1.3 “Deficiency” leads to the failure to fix the ramus and the loss of blood control. Liu Lao believes that “spleen deficiency, kidney deficiency and deficiency of both spleen and kidney are the most common causes of bleeding”. In Chinese medicine, it is said that “the spleen is the master of blood control”. A deficiency of the spleen, a disorderly diet, or excessive labor and fatigue can damage the spleen qi and cause the spleen to sink, resulting in the lack of consolidation of the ramifications and the loss of blood control. As stated in “The Essentials of the Gynecological Approach of the Golden Guide to Medicine”: “There are also those who are worried and hurt the spleen and whose spleen is deficient and unable to take in blood; those whose middle qi is sunken and unable to fix blood;”. Insufficient congenital kidney qi, weak kidney qi in young girls, gradually decaying kidney qi during the perimenopausal period; or early marriages and multiple births, indiscipline in intercourse, etc., which damage kidney qi, can lead to kidney sealing failure, and the punching of the blood can not be fixed, so that the menstrual blood cannot be restrained, and then becomes a leakage. 2.Explanation of the formula Liu has accumulated decades of theoretical research and clinical practice, and has closely followed the characteristics of the pathological mechanism of “deficiency, stasis and heat” of menorrhagia, and based on the principle of “those who are deficient should be tonified, those who remain should be attacked, and those who are hot should be cleared”, he has established a general formula to eliminate stasis and clear heat, and to benefit qi and nourish yin. This formula is used to stop bleeding. The main components of the formula are: Yi Mu Cao, Xian He Cao, Dang Ginseng, Shu Di Huang and Sheng Pu Huang. The formula is derived from the method, and the method follows the evidence. The prescriptions of Liu Lao’s ideas and treatment methods are all evident in the grouping of medicines. There is a saying in Chinese medicine that “if we see blood, we should treat it and eliminate blood stasis first”, and Liu also agrees with the view that “in all blood diseases, it is always important to eliminate blood stasis”, and it is crucial to activate blood and eliminate blood stasis, but we should also follow the principle of discriminatory treatment. Therefore, in the general formula for stopping bleeding, Yi Mu Cao is the ruling medicine. It is good at entering the heart and liver blood, activating blood circulation, regulating menstruation, eliminating blood stasis and creating new blood. It is said in the Compendium of Materia Medica that it can “invigorate the blood and break it up” and treat “leakage in collapse”. Pu Huang is good at invigorating the blood and eliminating blood stasis, cooling the blood and stopping bleeding. The “Justice of the Materia Medica” says that it “can guide stasis and cure stagnation of Qi and blood”. Panax pseudoginseng powder enters the blood part of the liver meridian and has the characteristics of stopping bleeding without leaving stasis and resolving stasis without injuring the righteousness. The New Compilation of Materia Medica praises it as “a miracle medicine to stop bleeding”. The two flavors help Yimu Cao to remove blood stasis and create new blood, treating the root of the problem and playing the effect of “plugging the flow” with “Tong”. The herb is used to stop bleeding, and is used to treat “hemorrhage” as described in “The Mirror of the Herbs”. Squid bone is a sentient product of flesh and blood, and has the function of astringing new blood and breaking blood stasis. The treatise on medicinal properties states that it is used to “stop bleeding in women”. The two drugs together, not only astringent to stop bleeding to cure its surface, but also help the ruling medicine to stop bleeding and strengthen the function of plugging the flow. As Li Shizhen said: “Use sour and astringent medicine to astringent its dissipation”. The five herbs are used to pass through and stop the flow, taking into account both the symptoms and the root cause, so as to achieve the effect of removing blood stasis and creating new ones, and stopping bleeding without leaving stasis! 2.2 Tonicity in attack, taking into account both deficiency and reality. Liu Lao believes that “deficiency, stasis and heat” are not the only causes of disease, but are often mixed with each other and are the cause and effect of each other, with a mixture of deficiency and reality. Purely eliminating blood stasis is easy to deplete qi and blood; purely supplementing deficiency has the disadvantage of “keeping invaders behind closed doors”. Therefore, the medicine should be dialectic, do not commit the precept of deficiency. The formula is supplemented with Radix et Rhizoma Ginseng and Radix Astragali. The party ginseng can not only replenish the spleen and lung Qi, but also replenish blood and generate fluid. The “Justice of the Materia Medica” its: “strengthen the middle qi, the original and ginseng is not very far”. Astragalus has the function of tonifying the qi and strengthening the spleen, raising the yang and lifting the trap, and the “Medical Keizhong Ginseng and Xilu” says that it: “can tonify the qi, and can also raise the qi, good at treating the chest in the atmosphere (i.e. zong qi) trapped”. When activating the blood to remove blood stasis, Wu tonifies qi and enhances the flavor of yang, on the one hand: qi is the handsome of blood, qi is sufficient to push blood powerfully, blood can be alive, spleen qi is strong to regulate power, blood can be stopped. As “Zhang’s Medical General” said: “gas and blood are attached to each other, gas is not blood, it is consumed and no unity; blood is not gas, it is condensed and does not flow”; on the other hand can make the tangible blood in the invisible gas, blood has a source. Such as “Fu Qing master of female medicine” cloud: “If you do not urgently replenish its gas to produce blood, but first replenish its blood and gas left behind, the tangible blood fear can not be born, and the invisible gas will be and to the end of dispersion, this is why not first replenish the blood and first replenish the gas.” 2.3 Prevent the disease before it happens and prevent the disease from changing. Liu Lao believes that stagnant blood is easy to generate heat and fire, and the evil of fire and heat is easy to torment Yin blood into stasis. The herb can not only activate the blood and move the qi, but also cool the blood to stop bleeding. It is also used as a blood cooling agent to stop blood stasis. The “Justice of the Materia Medica” states that “the herb is cold in nature and is used to treat blood heat and blood loss”. Amaranthus is clear of heat and cools the blood. The Compendium of Materia Medica says that it “disperses blood and subdues swellings, induces laxity in the intestines, and detoxifies the lymphatic system”. On the one hand, it can remove blood stasis and heat, and on the other hand, it can remove the cause of blood stasis and prevent the disease before it occurs. On the other hand, we can follow the method of “clearing heat” to suppress its boiling momentum, so that the sea of blood will be tranquil and the blood will not be overflowed if the heat is cleared. As stated in Ji Yin Gangmu: “In stopping astringency, it is necessary to contain the cooling, and to break the stasis and untie the knots”. 2.4 The use of Shu Di Huang. Liu Lao believes that blood is Yin, and blood and fluid are of the same origin, and blood loss also depletes Yin, as stated in the Treatise on Blood Evidence: “Blood is Yin, and blood deficiency means water deficiency. Among the many blood evidence, Liu made good use of Shu Di Huang. (1) Shu Di Huang is sweet in taste and is an important medicine to nourish blood and nourish deficiency. As the Materia Medica Justice says: “The deficiency of yin and blood in all the meridians, not rehmannia.” (2), Shu Di is good at nourishing kidney yin, so that the menstrual water has a source of transformation, “Medical Qiyuan” contains its “kidney water true yin deficiency”; (3) long collapse, long leakage, depletion of gas and injury to blood, deficiency heat, as “Fu Qingzheng master of women’s medicine” said: “people do not say that the fire is full of blood, but this fire is not real fire, is a false fire ear “. (4) follow the method of stopping the collapse in “Fu Qingzheng masters of women’s science”: “The medicine to stop the collapse should not be used alone, but must be used to stop the collapse in the Yin tonic”. (5) Helping the ruling medicine to invigorate blood circulation and resolve blood stasis, which is said in “The Pearl Pouch” to “open the blood vessels”. 2.5 Discriminatory treatment, with reference to the West. On the basis of the general formula for stopping bleeding, Liu added and subtracted on the basis of the evidence, and took into account both the symptoms and the root cause to shorten the course of treatment. In case of low hemoglobin and deficiency of qi and blood, he often adds ginseng to nourish the vital energy; in case of obvious heat, he adds nourishing products such as lady’s mantle and Mo dry lotus; in case of thick endometrium, he focuses on invigorating the blood to remove stasis, so that the stagnant blood can be removed and new blood can be produced; in case of thin endometrium, he focuses on astringent to stop bleeding and nourishing the kidney to stop the blood and produce qi and blood. In the case of acute bleeding period of uterine bleeding with large volume and urgency, we should follow the principle of “treating the symptoms when it is urgent” and use Chinese medicine combined with sex hormone program to avoid scraping and so on to reduce patients’ pain. 2.6 Holistic concept, pay attention to psychological regulation. The liver is the precursor of women, so if the mood is not smooth and liver qi is not comfortable, it will easily lead to rebellious qi and excessive drainage, or if liver depression turns into fire, forcing blood to move, it will also lead to leakage. As stated in “Fu Qingzheng’s Women’s Science”: “The liver is anxious, and if the qi is knotted, the qi is more anxious, and if it is more anxious, the blood cannot be hidden, so the collapse cannot be avoided”. With the development of society, the pace of life accelerates, the work pressure increases, increasing the occurrence of this kind of emotional disease. Liu paid great attention to the psychological guidance of the disease, so that the liver Qi Shu, Qi smooth, blood to the liver, the punching Ren to solid. Liu Ruifen and others found that before the psychological factors were eliminated, the effect of drug treatment alone was not good. Through long-term exploration, they found that psychotherapy, i.e., reasoning and suggestion, together with appropriate medication and acupuncture, was effective. Liu also attaches importance to the overall treatment after healing, advising patients to have a reasonable diet; to develop good habits of work and rest; to communicate with family and friends to maintain a relaxed mood; to combine work and rest, and to exercise moderately to enhance physical fitness.
