The patient, XX Huang, female, 41 years old, hospitalization number 11391, presented to the clinic on August 23, 2013 with the chief complaint of “incomplete menstruation for 17 days”. The patient had her menses on August 06, 2013, and at the time of consultation, she was still dribbling incessantly, with a small amount of black blood clots, accompanied by lumbago and abdominal pain, mental fatigue, dizziness and palpitations, shortness of breath on movement, swelling of both lower extremities, poor appetite and sleep, and a regulated bowel movement. The tongue is fat, light red in texture, with thin white coating and weak pulse. Menstrual history: 14 10-12/26-28, LMP2013.08.06, medium volume, dark color, with blood clots. Fertility history: 1 son and 1 daughter, spouse and children are healthy. Auxiliary examinations: routine blood and five coagulation items were generally normal; ultrasound (hepatobiliary, pancreatic and spleen, uterus, both adnexa and bladder) showed: 1. multiple slightly hyperechoic clusters in the liver, nature to be determined 2. anechoic area of the left ovary, nature to be determined 3. no obvious occupying lesions in the biliary, pancreatic, spleen, bladder, uterus and right adnexa. A frontal and lateral chest radiograph shows a round nodular shadow in the right lung, the nature of which is to be investigated. Diagnosis: Leakage – Qi deficiency and blood stasis. Treatment: Treat the symptoms urgently and treat the root cause slowly; stop the flow and stop the bleeding mainly, and clarify the source to treat the root. The effect is not to change the formula, continue to take it according to the original prescription. 2013.08.29 Menstrual bleeding basically stopped, with only a small amount of yellow discharge, a little odor, slight weakness, transient palpitations, difficulty in sleeping, short sleep time. The tongue is fat, light red in quality, with thin white coating and thin pulse. He continued to take three doses according to the above, and was discharged with all the symptoms removed. In the seventh year of life, the kidney energy declines, the natural sebum is gradually depleted, and the essence and blood become deficient, so the changes of bleeding at this time are mostly caused by deficiency of positive energy, disharmony of yin and yang, and loss of solidity of the ramifications. However, the deficiency of both spleen and kidney is the essence of the deficiency of vital energy in the aging organism, and deficiency for a long time will lead to stasis. If Yang Qi is deficient and Yin cold is strong, it can lead to coagulation of blood vessels; if Yin blood is deficient, the veins are dry and astringent, or if Yin deficiency and fire are strong, the blood is boiled, which can also lead to stagnation of blood vessels. The kidneys are the essence of the first heaven, where the yin and yang are hidden, once deficient, it will lead to stagnation of blood vessels. If the spleen and stomach are deficient, there is no source of biochemistry, and the deficiency of qi will lead to blood stasis. Therefore, as the body grows older, the spleen and kidneys become gradually deficient, the qi and blood gradually decline, and their operation gradually slackens and even clots, which also indicates the existence of potential blood stasis in the aging process of the body. Some scholars have found that the detection of blood stasis evidence increases with age. In addition, according to the theory of blood stasis in Chinese medicine, “blood loses its root, and blood stasis is its root”. Once the blood stasis is formed, it stays and does not go away, and the blood stasis blocks the uterus, resulting in new blood not being produced and the blood not returning to the menstruum, thus leading to the leakage in the menopause.
