The occurrence and severity of radiation pneumonia are closely related to the method of radiation, the amount of radiation, the area of radiation, and the speed of radiation. The symptoms of radiation pneumonia are asymptomatic in mild cases, and most of them appear 2-3 months after radiation treatment. Most of them have symptoms 2-3 months after radiation treatment, and some of them have symptoms half a year after stopping radiation treatment. Common symptoms: irritating dry cough, shortness of breath, increased after activity, chest pain, with or without fever, more often with low fever, swallowing difficulties when causing radiation esophagitis, severe respiratory distress and cyanosis in severe cases. Atrophic hardening of the skin is seen locally in the chest radiation. There are no positive signs on examination of the lungs in most cases. When extensive pulmonary fibrosis is present, alveolar breath sounds are generally diminished and twisted sounds can be heard. In case of secondary bacterial infection, dry and wet rales can be heard. Occasionally, pleural friction sounds are present. In case of concomitant pulmonary heart disease, signs of right heart failure may be present. The main treatment for radiation pneumonia is heat toxicity, yin deficiency, blood stasis, and qi deficiency; benefiting qi and nourishing yin, clearing heat and detoxifying toxins, and activating blood stasis. The drugs are commonly used in the treatment of radiation pneumonia. In addition, the phlegm-transforming herb Bayberry, the blood nourishing herb Shengdi and Agaricus are also commonly used in the treatment of radiation pneumonia. The application of the method of benefiting qi and nourishing yin, clearing heat and detoxification, activating blood circulation and resolving blood stasis accounts for 67% to 82%, which is the first major method, and the method of generating fluid and moistening dryness, relieving cough and resolving phlegm is the secondary treatment for radiation pneumonia. Yin injury and lung dryness Most commonly seen 1 to 3 months after radiotherapy, mainly manifesting as irritating dry cough, no or little sputum, sore throat, dry mouth and cold drinks, chest tightness and heartburn, or with low fever, poor appetite, red tongue with little coating and lack of fluid, and fine pulse. The treatment is to nourish Yin and clear heat, moisten the lung and promote the production of fluid, using Sha Shen Mai Dong Tang and Qing Dryness and Save the Lung Tang with addition and subtraction. Lung Spleen-Qi Deficiency with Blood Stasis The patient has a deficiency of the spleen, combined with radiotherapy injury, the disease is prolonged, with recurrent coughing episodes. The treatment is to tonify the lung and strengthen the spleen, dispel dampness and eliminate blood stasis by combining ginger and licorice soup with Er Chen Tang. Heat toxicity, phlegm and heat in the lung. After radiation therapy, vascular permeability is increased, alveolar interstitial edema, and easily combined with pulmonary infection, resulting in internal heat toxicity and phlegm fire. The symptoms include fever, cough with thick or thick yellow sputum, coughing and vomiting, chest pain or coughing blood, dry mouth and desire to drink, red tongue, thin yellow or yellowish coating, and slippery pulse. The treatment is to clear heat and detoxify the toxin, clear the lung and resolve phlegm, using Qing Jin Hua Phlegm Tang and Qian Jin Wei Zhi Tang with addition and reduction.
