Chinese medicine rehabilitation for tumor patients

1, the current situation The five organs and six bowels of the human body are the basic structure for maintaining life activities. They absorb water and grain essence, run qi and blood and transport fluid, so that the human body constitutes an organic whole. The dysfunction and deficiency of internal organs can often lead to the occurrence of tumor and affect the health of the body. The sequential or combined application of various treatment methods can also cause further deficiency of the human body. In ancient times, Li Zhongzi, a famous doctor, put forward the theory of “nourishing the righteousness and eliminating the accumulation since”, which coincides with the idea of modern medicine of fighting tumors through immune regulation. In view of the characteristics of tumor patients’ disease mechanism of “positive deficiency and evil solidity”, TCM tumor rehabilitation takes the support of positive as the main method of treatment, which runs through the whole course of rehabilitation treatment. According to the characteristics of different treatment stages, in order to minimize the damage caused by the treatment, the TCM rehabilitation methods include helping the righteousness, reducing the toxicity and protecting the evil: according to the characteristics of different treatment stages, in order to help the righteousness and reduce the toxicity, and further harmonize the yin and yang of the internal organs and the qi and blood; after each treatment stage, the method of helping the righteousness and protecting the evil is used to promote the body’s recovery and resist the recurrence and metastasis of tumor. There are many Chinese medicine rehabilitation methods for tumor patients, which roughly include: oral tonics, oral Chinese medicine, nourishing creams, external treatment with Chinese medicine (compress, soak, fumigation, etc.), intravenous injection, acupuncture, massage and so on. In addition to these traditional methods, we also combine psychological intervention, exercise, nutritional intervention and other integrated methods to develop individualized rehabilitation programs for patients. The national “Tenth Five-Year Plan” and “Eleventh Five-Year Plan” projects that we have undertaken have proven the efficacy of Chinese medicine. We hope that lung cancer patients who meet the enrollment criteria can actively join the group and reap the benefits of cancer patient rehabilitation. Inclusion criteria for lung cancer rehabilitation enrollment: (1) Patients with pathologically or cytologically confirmed non-small cell lung cancer; (2) Patients who have completed 3-4 months of postoperative adjuvant chemotherapy after surgery for stage III NSCLC; stage I-II patients who do not require postoperative adjuvant therapy after surgery; (3) ECOG score:0-1, KPS score 80-100; (4) Expected survival ≥ 6 months; (5) ) Age 18-75 years; (6) Signed informed consent and good compliance. In fact, due to the long history of TCM in China and the accumulated clinical experience, the proportion of our people adopting TCM as a means of treatment and rehabilitation for oncology patients is also very high. Almost all patients have taken or are taking TCM or proprietary Chinese medicine. Even in western hospitals, doctors will prescribe proprietary Chinese medicines for patients at their discretion to nourish blood, support righteousness, regulate immune function, improve clinical symptoms, etc. 2.Misconceptions (1) Chinese medicine treatment can only be started after radiotherapy is finished Explanation: Rehabilitation of tumor patients should run through the whole treatment of tumor patients, and intervention should be started from the time of diagnosis. TCM treatment and rehabilitation is an important part of comprehensive treatment for tumor patients, which plays different roles in different stages. The earlier Chinese medicine treatment is started, the more conducive to patients’ successful completion of western medical treatment, and the more conducive to rehabilitation. (2) Traditional Chinese medicine and radiotherapy have conflicts Explanation of misunderstanding: Some western doctors do not understand traditional Chinese medicine, or their understanding of traditional Chinese medicine comes from charlatans or advertisements, which misinterpret traditional Chinese medicine and even mislead patients. Many patients attending outpatient clinics come to consult with anxiety, saying that Western doctors do not allow Chinese medicine and are afraid that Chinese medicine will conflict with chemotherapy and radiotherapy. In fact, during the radiotherapy phase, Chinese medicine usually focuses on supporting the righteousness of the patient, and according to the different deficiencies of the patient, such as blood deficiency, qi deficiency, and yin deficiency, Chinese medicine is used to support the righteousness of the patient and strengthen the patient’s constitution, and at the same time, according to the side effects of radiotherapy, the patient’s gastrointestinal function is regulated in a targeted manner to reduce the gastrointestinal reaction, promote bone marrow hematopoiesis, and regulate immunity. Many of the patients we have seen in the radiotherapy stage, many of whom came to us with the intention of interrupting or abandoning their treatment because of the side effects of radiotherapy, were able to receive treatment and complete the course of treatment with the help of Chinese medicine. Chinese medicine treatment should also be standardized. Therefore, last year we formulated the guidelines for Chinese medicine tumor treatment, which is to standardize the treatment rules and methods of Chinese medicine in the treatment of tumor patients and to guide clinicians to standardize treatment. It is suggested that tumor patients patients should go to qualified and experienced TCM oncologists when seeking TCM treatment, and should not blindly believe in advertisements so as not to delay their illness. (3) Surgery has been removed cleanly, and even radiotherapy is not needed, let alone follow-up TCM treatment. Malignant tumor has the risk of metastasis and recurrence, so it should be reviewed regularly and a comprehensive rehabilitation method should be actively adopted to promote the recovery of qi, blood, yin and yang and the balance of organ functions. If the previous bad lifestyle is not changed, the bad psychological state is not adjusted and the deficiency in the body is not restored, malignant tumor may come back. Therefore, in time, patients who have been cured by early surgery should also be vigilant and adopt scientific rehabilitation methods to actively adjust their physical state and strive for ultimate recovery. (4) With TCM rehabilitation, there is no need for Western medical treatment. At different stages of treatment, depending on the patient’s physical condition, tumor condition, and the severity of the disease, one can choose Western medicine treatment as the main treatment and Chinese medicine treatment as a supplement or Chinese medicine treatment as the main treatment, follow up and observe, and once the condition changes, one should re-evaluate and choose a suitable combined Chinese and Western medicine treatment strategy. Any propaganda that amplifies the advantages of TCM or Western medicine or amplifies the disadvantages of TCM or Western medicine is not scientific and is not conducive to the treatment and recovery of patients and is irresponsible. We, as doctors, should adopt the best treatment in the interest of patients to help them alleviate their pain and recover their health, and should not reject both Western and Chinese medicine. 3. Domestic and foreign comparison The rehabilitation of tumor patients in foreign countries started earlier, but due to the lack of experience and conditions in the application of TCM, most of them choose psychological intervention, phytotherapy (extracting active ingredients from plants and making health products), dietary supplements, aerobic exercise, yoga, meditation, music therapy, aromatherapy and so on when they carry out the rehabilitation of tumor patients. We have exchanged and studied with oncology patient rehabilitation doctors at Mayo Center in the United States. They mainly use telephone follow-up to guide patients to perform functional exercises after surgery, and for patients with psychological problems or nutritional problems, intervention and management are carried out by specialists. We use Chinese medicine treatment (including oral Chinese medicine and external treatment), acupuncture, psychology (including psychotherapy, meditation, music therapy, aromatherapy, etc.), traditional exercise, nutritional intervention and other integrated methods in the rehabilitation of tumor patients, i.e., through a rehabilitation team composed of various types of doctors to provide individual rehabilitation guidance for each patient, and the use of this form of systematic rehabilitation training and guidance for patients is unique in It is unique in China. 4. How to regulate The regulation of Chinese medicine in the treatment and rehabilitation of tumor patients is carried out in stages. (1) Chinese medicine combined with surgery Surgery can eradicate most of the early stage tumor lesions, but it is more traumatic and due to the spread of tumor metastasis, radiotherapy is still needed after surgery to treat metastasis and consolidate the efficacy. Palliative treatment of advanced tumors sometimes requires surgery in combination with radiotherapy. The trauma of surgery coupled with the adverse effects of radiotherapy, while treating the tumor, the patient’s body should not be underestimated, which requires the systemic conditioning of Chinese medicine. Before surgery, Chinese medicine is used to support the righteousness, improve the patient’s nutritional status and strengthen the body to lay the foundation for the surgery; after surgery, Chinese medicine is taken to regulate the spleen and stomach, nourish Yin and produce fluid, improve the quality of life and promote the recovery of the body. (2) Chinese medicine combined with radiotherapy Radiotherapy has always occupied a dominant position in tumor treatment, but radiotherapy, while combating tumors, will affect the digestive function, injure the spleen and stomach, causing patients to experience nausea and vomiting, and can affect the operation of qi and blood causing dizziness and fatigue, etc. It will also cause damage to liver function and reduce the body’s immune system. The use of Chinese medicine during radiotherapy reduces the side effects of radiotherapy and ensures the smooth implementation of radiotherapy. Chinese medicine can regulate the function of the spleen and stomach, improve sleep, enhance physical strength and improve immunity, thus enhancing the therapeutic effect. (3) Advanced patients Many advanced patients, as the internal organs and yin and yang are already seriously imbalanced, undergo radiotherapy and surgery, which often aggravate the imbalance and lead to adverse consequences. At this time, patients can only use TCM treatment, which is guided by a balanced viewpoint, and the patient’s organism can reach a relative stability, and thus can survive with the tumor for a long time, and the quality of life will be better. (4) Recovery period, avoiding and reducing tumor recurrence and metastasis According to Chinese medicine, “positive does not suppress evil” is the key to tumor recurrence and metastasis. After radiotherapy and surgery, it is not guaranteed that all the tumors in the body are removed, plus the weakened immunity of the body after treatment, the positive and the evil in the body are still confronting each other, when the residual tumor pathogenic force wins, the disease develops and the tumor recurs and metastasizes. Therefore, during the recovery period of tumor patients, taking Chinese medicine to help the positive and eliminate the evil, enhance the body’s immune ability and resist the recurrence and metastasis of tumor, so that some patients with residual tumor lesions can still survive for a long time and have a good quality of life. Summary: Chinese medicine treatment should be a reflection of “orderly treatment” and “holistic treatment”, that is, according to the specific conditions of patients, different treatment strategies should be adopted in different stages. The advantages of Chinese medicine in treating tumors and tonifying the body are as follows: (1) post-surgical patients: due to surgical damage, patients mostly show deficiency of both qi and blood, often showing symptoms such as weakness, spontaneous sweating, night sweating, dullness and abdominal distension, insomnia and dreaminess, etc. Using Chinese medicine can nourish qi and blood, reduce surgical complications, and facilitate patients’ faster recovery; (2) through Chinese medicine to support the righteousness and dispel evil, consolidate the therapeutic effect, and reduce tumor recurrence and metastasis (3) Taking traditional Chinese medicine during radiotherapy and chemotherapy can alleviate the side effects of radiotherapy and chemotherapy such as nausea, vomiting, constipation, leukopenia, anemia, insomnia, pain, dry mouth and thirst; (4) Patients with advanced stage or lesions not suitable for surgery or radiotherapy and chemotherapy treatment: taking traditional Chinese medicine can control tumor growth, alleviate symptoms, improve quality of life and prolong survival.
