Acupuncture is well known for its superior efficacy in treating pain. Acupuncture can treat almost all kinds of pain, and its effect on pain can reach the degree of immediate effect. The author believes that the treatment of pain by acupuncture must be guided by the theory of Chinese medicine – acupuncture and combined with human physiological anatomy and pathology in order to improve the therapeutic effect. The common clinical neck, shoulder, waist and leg pains are mostly called “paralysis (pain paralysis)”, “cold paralysis”, “wet paralysis” and “tendon paralysis”. “paralysis of tendons” and so on. These pains are mostly caused by acute or chronic injuries, or by aseptic inflammatory adhesions of soft tissues caused by dampness and wind and cold. Fibrous tissue hyperplasia, inflammatory tissue degeneration and contracture are caused. In the diagnosis, clinical symptoms and signs should be considered first. Instead of relying only on the bony changes on X-ray film as an important basis for diagnosis, it is more important to examine the lesion by palpation and accurately determine the pressure and pain points at the lesion site. The treatment principle should be to activate the meridians, relax the tendons and blood, loosen the contractures and eliminate the inflammatory adhesions, and relieve the pain as its purpose. The principles of acupuncture and analgesia 1. Combination of local and distal acupuncture points: “The main treatment of the meridian is to be taken by the meridian”, so the pain is often taken locally and by the relevant transmission points on the meridian, which are divided into near and distal acupuncture points. For example, for toothache, cheek car and shimonoseki can be taken near, while hegu and nei ting can be taken far away; for caesarean section, belt veins can be selected locally, and san yin jiao can be taken distally; for migraine, rate valley, sun, fengchi and jiao sun can be taken locally, while wai guan and yangling quan can be taken distally; for posterior head pain, luo but, yu pillow, fengfu and fengchi can be selected locally, while kunlun and houxi can be taken far away. In addition, in addition to the local acupuncture points, great attention is paid to the use of acupuncture points in the local pain “pain for loss” to obtain more obvious analgesic effect. Such as frozen shoulder, humeral epicondylitis is often selected pressure pain points for acupuncture. 2. Combination of evidence-based acupuncture points and disease-based acupuncture points: evidence-based acupuncture points are selected according to the clinical manifestations of the patient’s disease using visceral and meridian identification. For example, in general stomach pains, the foot Sanli, Neiguan, and Zhonghua; if the stomach pain is identified as liver qi offending the stomach, or chest distension and pain, Taichong can be taken to dredge the liver and relieve the liver; for swelling and pain in the eyes, Xingma and Manxi are often taken; for biliary tract diseases, Bile Yu is used. In addition, in clinical acupuncture analgesia is also often taken according to the disease identification points, such as gastric and epigastric pain regardless of the type of evidence, often choose Liangqiu; cholecystitis, appendicitis often choose the gallbladder points and appendix points; pillow with the pillow points, lumbago points, lumbago points, Wei Zhong points, etc.. 3. Combination of empirical and experimental acupuncture points: After long-term clinical practice, each family has accumulated a lot of experience in acupuncture analgesia, so there are many empirical points commonly used for analgesia in clinical practice. For example, the blood is released from the tip of the ear for red eyes and swelling pain; the blood is released from Shaoshang for swelling pain in the throat; Renzhong or Houxi through Hegu for acute lumbar sprains; Tianzong and Tianrong for biliary colic, etc. The analgesic effect of these infusion points may not be directly related to general meridian theory or visceral theory, but comes from a large number of clinical practices, so they are called empirical points. In addition, some of the clinical points used for analgesia come from experimental studies, and although these points often have one or another theoretical basis before the experiments are conducted, they are now widely used based on their experimental results, so they are called experimental points. In modern experimental acupuncture, although the experimental subjects are mainly animals, it has a greater clinical impact. Since the mechanism of acupuncture analgesia is multifaceted, this type of infusion point is widely used for a variety of painful diseases, and is not limited to which diseases or which symptoms. For example, Sanyinjiao, Neiguan, Sanyangluo, Renzhong, Hegu, Shusanli, and Zhizhong, etc. For example, experiments have shown that acupuncture of Sanyinjiao and Sansili can significantly increase the pain valve in traumatized rats, indicating that the infusion point has analgesic effects and a longer post-acupuncture effect. Another example is that some experiments have shown that electro-acupuncture points such as Yanglingquan, Feet Sanli, Zhizhong and Huanjiao have better analgesic effects, especially the inhibitory effect of electro-acupuncture point Zhizhong on cold-induced persistent pain can reach 48 h, which is better than other experimental transfusion points, also indicating that the choice of acupuncture points is one of the important factors affecting the analgesic effect of acupuncture. Second, pain 1, limb pain Limb pain mainly refers to pain in the muscles and joints of the upper and lower limbs. The nature of pain has various manifestations, such as string pain, heavy pain, button pain, swelling pain, stabbing pain and so on. Treatment should still focus on meridian identification, according to part of the meridian, with acupuncture points following the meridian. Depending on the scope, location and characteristics of the pain, acupuncture points can be taken differently. (1) Pain on one side, disease in one meridian, manifesting as string pain along the meridian, most of the neuralgia in Western medicine belongs to this category. According to X-ray or CT localization method, Huatuo pinch points can be taken from the lesion area to relax the local tense muscles, restore the normal intervertebral joint position, loosen the adhesions inside and outside the vertebral canal, improve the local blood supply, and achieve the purpose of swelling and analgesia. You can also follow the meridians to select 1-3 points with better needle sensation conduction, such as Quchi, Hegu and Jiquan for the upper limbs; and Huanjiao, Foot Sanli, Yanglingquan and Weizhong for the lower limbs. (2 ) If the pain is in one place and the disease is limited, it is mostly a meridian tendon lesion. If the pain is acute, local treatment will not remove the stagnation, but will aggravate it. It is only appropriate to use the method of spreading, moving Qi and activating Blood. The main method is to take the distal points along the meridian and use strong stimulating needles to bring the qi to the disease, which is often effective. If necessary, the method is combined with localized acupuncture points or ayurvedic acupuncture points to perform shallow stabbing. The corresponding acupuncture point method is used, including left to treat the right, right to treat the left (giant acupuncture, mu acupuncture), above to treat the lower, below to treat the upper (wrist and ankle correspondence, elbow and knee correspondence, shoulder and hip correspondence), before to treat the back, after to treat the front, etc. If the pain is chronic, it is mainly due to local astringency of qi and blood and stagnation of the ligaments and veins, so local acupuncture points should be taken mainly, together with distal acupuncture points along the meridians to unblock them. (3) Pain in the peripheral limbs and several meridians. If the pain is caused by depressed liver qi, take Hegu and Taichong to open the four gates to move the qi and stop the pain; if the pain is caused by loss of harmony between qi and blood, take Quchi and Shu San Li to regulate the qi and blood; if the pain is caused by the tendons, take Yanglingquan and Wai Guan to relax Shaoyang and relax the tendons and activate the channels. If the pain is indefinite, and the movement is variable, it is caused by the wind and qi, so take the points of qi tonification to achieve the effect of “the wind will be extinguished when the blood is moving”. Some limb and joint pain, the effect of long-term treatment is weak, you can try to take the two channels. Taking the Governor’s Vessel can regulate the qi of all the yin and yang, of which the upper body pain is dominated by the dazhi and the lower body pain is dominated by the vital gate. Taking the Ren channel can help the essence of all yin, such as Guan Yuan, Qi Hai and Shen Que. For those who suffer from mental depression and complain of body pain, attention should also be paid to taking head points and back points to calm the mind and relieve depression and pain. For example, Shen Ting, Si Shen Cong, Xin Yu, Liver Yu and Spleen Yu. 2, head and facial pain Head and facial pain refers to pain caused by diseases of the five senses and various other causes. According to the theory of organs and meridians, the five senses of the head and face are closely related to the specific internal organs and meridians in physiopathological terms. Generally, the points below the elbow and knee joints should be used, and the internal organs should be treated by taking the dorsal or spinal points. In addition, it is advisable to take the points based on the division of the meridian combined with the identification of the evidence. 3. Torso pain Torso pain refers to pain in the chest, abdomen, back and waist. It should be identified by the meridians and taken according to the part of the meridian, far and near. For example, if the dorsal pain is blamed on the liver and gallbladder channels, it is advisable to take Tai Chong, Yanglingquan and Zhigou from afar and Zhangmen and Dabao from near. In addition, acute pain should be taken from afar, while chronic pain should be taken locally. 4. Visceral pain Visceral pain includes pain caused by lesions in the thorax, abdomen and pelvic organs. The following aspects should be considered when forming prescriptions. (1) Acute visceral pain. Most of them are actual evidence or evidence of deficiency and symptoms, with rapid onset and severe pain. To pay attention to the identification of the disease and treatment, to “treat the symptoms” as the method, in order to quickly stop the pain as the purpose. Use localized points, Qie points, Hepa points, and Luo points that can quickly correct Qi and blood rebellion. (2) Chronic visceral pain. Mostly deficiency or mixed evidence of deficiency and real fire, with a long duration of illness and slow and continuous pain attacks. Treatment is based on the principle of “treating the root of the problem when it is slow” and “taking into account both the symptoms and the root of the problem”, using more Yu points, collection points and original points. (3) Visceral pain is caused by the loss of harmony between the qi and blood of the viscera, so the selection of acupuncture points should not only focus on the identification of the viscera and meridians, but also combine with the etiology of the disease, using a variety of identification methods such as identification of qi, blood and fluid, and identification of the six meridians. Regardless of the urgency and severity of the pain, specific points should be selected for their specific therapeutic effect on the pathogenesis. For example, in case of cold condensation, Guan Yuan and He Gu are taken; in case of fire and heat, each of the meridian Xin Xin points are taken; in case of Qi stagnation and blood stasis, He Gu and San Yin Jiao are taken. (4) Visceral pain is often manifested as chest, abdomen and back and waist traction pain. For this, even stabbing is effective. The Yu Reciprocal Point Method is a typical even-prick method. In addition to traction pain in the corresponding Yu points, acute and chronic visceral pains often show discoloration, depressions, protrusions, subcutaneous nodules, and pressure pains around the dorsal points, and these positive reaction points can be searched for and needled with good results. As mentioned above, when taking acupuncture points for pain, the principle is to take the points according to part of the class, according to part of the meridian, and following the meridian. In addition, when prescribing acupuncture points, attention should be paid to both treating the mind and moving pain. You can use Si Shen Cong, Bai Hui, Shen Ting, Ben Shen, Shen Men, Neiguan, and Xin Yu, which nourish the heart and calm the mind.
