How is polycystic ovary syndrome diagnosed?

Polycystic ovary syndrome is a common gynecologic endocrine disorder of reproductive age that may present with menstrual disorders, acne, hirsutism, and ultrasound suggestive of polycystic ovarian changes. The diagnosis of polycystic ovary syndrome relies on three main criteria: sparse ovulation, manifestations of elevated androgens, and ultrasound findings, as well as the exclusion of other similar conditions. Corresponding investigations include a detailed menstrual history, physical examination (height and weight, presence of acne hirsutans and acanthosis nigricans), blood sampling for ovarian hormones + thyroid function + adrenal-derived androgens + glucose tolerance test + insulin release test on days 2-4 of menstruation, pelvic ultrasound on days 6-8 of menstruation, and monitoring of ovulation.
