In the early days of human civilization, the penis was not only a part of the human body, but also an idea and a symbol of male majesty. Those with short penises are perceived as lacking in manliness and creativity, and they can easily close themselves off, affecting their socialization and self-confidence. A statistic in the United States shows that about 2% of men believe that their penis is not large enough for sexual intercourse, and 62.7% of patients with short penis syndrome feel anxious during childhood, while 37. 3% of patients begin to feel anxious only during puberty. The exact method of endocrine treatment is controversial, and surgery is often required after puberty. Some studies have found that some adults, despite having average penile size, suffer from chronic anxiety, low self-esteem, and fear due to excessive concern about their penile size affecting their sexual satisfaction, even leading to erectile dysfunction and significantly lowering their quality of life. As a result of the increased attention to the genitals, a significant number of patients have developed the procedure from a reconstructive surgery to a cosmetic surgery simply to fulfill a psychological need. Nowadays, there are no recognized indications for penile lengthening to be carried out in China, and hospitals tend to decide whether or not to perform surgical treatment based on their own experience and the patient’s condition. However, from my personal long-term clinical observation and a large number of surgeries, the indications for penile lengthening surgery can be summarized as follows: 1, penile dysplasia; 2, erectile length <10cm; 3, penile trauma, partially dissected; 4, want to improve the sensation of the male during sexual intercourse; 5, engaged in certain occupations such as actors/models, etc.; 6, penile length of erection of up to 10cm, but still can not meet the female sexual requirements; 7, Those who have normal or low normal penile development, but are self-conscious of penile shortness, have a serious inferiority complex, and urgently require surgery. In addition to the indications, surgical safety and postoperative results are the content of common concern for patients. There are 3 problems that need to be solved in the lengthening of the penis: 1. How to release the hidden corpus cavernosum through the disarticulation of the superficial suspensory ligament and part of the deep suspensory ligament? 2, what to cover the trauma after releasing the corpus cavernosum? 3, How to prevent the released cavernous body from not retracting or retracting very little? The penis root consists of the funnel ligament attachment, suspensory ligament attachment and interligamentous part, and the total length is 7.67cm, which is the maximum length that may be released after dissecting the ligaments during the penile lengthening surgery. In penile lengthening and repair of most penile defects and other penile plastic surgery, cutting the funnel ligament and suspensory ligament, and stripping the anterior 1/3 of the penile pedicle from the pubic subpubic branch can release a greater penile length without affecting penile blood circulation and erectile function, and there is no significant effect on standing urination and penile stability. Generally speaking, the operation of the deep suspensory ligament of the penis to cut off its 1/3 ~ 1/2 enough, and do not have to cut it off completely, or even peel part of the cavernous foot of the penis, in order to maximize the extension effect, otherwise, although the appearance of the appearance of longer, but easy to cause the appearance of the penis to shift downward, the angle of erection is lower or even lower than 90 °, the stability of the penis erection decreased, on the contrary, it will affect the normal conduct of sexual life, thus failing to achieve the desired purpose of surgery. Thus, the expected surgical purpose cannot be achieved. After the superficial and middle suspensory ligaments are cut off, there is no ligament to pull the penis when it is erected, so the stability of the penis will inevitably be affected to a certain extent.
