Li Ye (李冶), character Jilan (季蘭), was a poetess and a poetess in the Kaiyuan, Tianbao and Dali periods. Her poems were so magnificent that later generations had the following words: her poetic style was both majestic and poetic, and there were few poets from Bao Zhao down. Li Jilan was so open-minded that she made friends with many talented people from all over the world and founded a literary salon in Kaiyuan Temple. At a salon gathering, Liu Changqing, known as “The Great Wall of Five Words”, was the most famous and highly respected, and many people surrounded him in front and back, but he had a “Yin heavy disease”, that is, a hernia. Li Jilan and he is an old acquaintance, some unexplained relationship, others that he as a dish, but she is not, to give him a hard time. Li Jilan was deliberately late and said to him that “mountain gas is good every day and night” to laugh at Liu Changqing’s hernia disease. The hernia is a harmonic of hernia, which means that Liu’s hernia has been better recently. Liu Changqing, on the contrary, laughed and immediately replied with a poem by Tao Yuanming: “The birds are happy to have a trust”. Liu Changqing meant that although the hernia was getting heavy, he was glad that there was still a way to relieve it. Li Jilan, instead of being angry and embarrassed, rejoiced and praised Liu Changqing for his courage, and even poured wine to persuade him to drink. The two of them used Tao Yuanming’s poem in their question and answer, and made fun of each other with harmonious sounds, which became a story of elegant banter at that time. The story is from Xin Wenfang’s “The Biography of Tang Talents”, in which Xin Wenfang wrote that Li Jilan was “so ridiculous” and his eyes were full of contempt. What exactly is hernia disease? Hernia is also called small intestine hernia, and the medical term is hernia, which is classified as umbilical hernia, inguinal hernia, hiatal hernia, incisional hernia, surgical recurrent hernia, white line hernia, femoral hernia, and so on. Simply put, hernia is a tear or hole in the muscle tissue of the abdominal wall, through which the patient’s small intestine and other organs or tissues leave their original position and protrude into other parts of the body, just like a car tire, the outer tire is broken and the inner tire protrudes through the hole; it is mostly caused by coughing, sneezing, excessive force, excessive abdominal fat, forceful defecation, women’s pregnancy, excessive crying of children, degenerative changes in the strength of the abdominal wall in old age, etc. caused. Clinical manifestations of hernia When a child has a hernia, a lumpy bump can be found in the groin when touched. This bump often appears when the child cries hard, exercises vigorously or is constipated, and disappears when the child lies down or when the hand is pressed. When a child has a hernia, the body’s digestive system is also affected, causing symptoms such as bloating, abdominal pain, and constipation. Some children with hernias can become very weepy and irritable, so parents must be alert. In adults with a hernia, a lump may also appear in the groin area. Whenever the patient coughs or defecates hard, the lump will protrude, and when lying down the lump will subside. When these masses are touched and squeezed, the patient may feel a slight sense of resistance and may hear a gurgling sound. Whenever the masses protrude, the patient’s genitourinary system is squeezed, so he or she may feel cramping and swelling in the lower abdomen, and sexual dysfunction may be significantly impaired in hernia patients. When an elderly person has a hernia, the site of prolapse is usually the small intestine, and in some patients, the site of prolapse is the large intestine or appendix. Therefore, body masses in the elderly are soft to the touch and a gurgling sound is heard when pressed. In addition to body lumps, elderly people with hernias may also suffer from common symptoms such as abdominal pain, bloating, constipation and indigestion. How is it treated? Since a hernia is caused by a defect in the abdominal wall, there are no medications that can treat an extra-abdominal hernia other than surgical repair. In young children under half an age, the hernia may be self-healing because the abdominal muscle gradually strengthens during development, so surgery can be suspended; in patients of advanced age or with serious underlying diseases who cannot tolerate surgery, the hernia ring can be blocked with an external hernia belt to prevent protrusion and further expansion of the hernia; in the rest of cases, surgery is recommended. Some strangulated hernias combined with severe infection may cause repair failure, so they can also be temporarily ligated and repaired surgically in the second stage. 2.Tension-free repair with filling: this method turns the part of the hernia ring, which was the lowest point of peritoneal stress, into the highest point instead, avoiding the hernia from protruding from here again and reducing the chance of recurrence. This method is easy to operate, does not require excessive freeing of the hernia sac, and has ideal postoperative results, and is currently the main method of tension-free hernia repair in some primary hospitals. This method is more demanding than the filling method and requires high freeing of the hernia sac up to the anterior peritoneal space. The advantage is that the patch is large and flat, which is more physiological and less uncomfortable for the patient, and the anterior peritoneal space can cover the whole pubic foramen area to prevent recurrence of hiatal, direct and femoral hernias. This is the most commonly used open tension-free hernia repair at present. 4.Laparoscopic hernia repair: With the increasing improvement of minimally invasive technology, laparoscopic hernia repair is becoming more and more widely performed and mature. Compared with open surgery, laparoscopic surgery starts from the posterior side of the hernia, i.e. the starting point of the hernia protrusion, so that the hernia protrusion can be observed more directly and the patch placed can be larger and flatter, which is a recommended surgical method at present.
