What’s that swelling under your eye?

A swollen bag under the eye may be related to blepharitis, blepharophalangeal cyst or sebaceous gland carcinoma of the eyelid.
1. Blepharitis: It is a purulent inflammation of the eyelid glands, also known as wheal, mostly caused by staphylococcal infection, and is divided into external blepharitis and internal blepharitis. External blepharitis may appear in the eyelid margin at the root of the eyelashes, with diffuse redness and swelling, and hard nodules may be palpable; internal blepharitis may appear as hard nodules with tenderness within the eyelid glands.
2. Blepharophalangeal cysts: their occurrence may be related to blepharitis or chronic conjunctivitis which leads to the obstruction of the outlet of the blepharophalangeal glands, thus the glandular secretions can be trapped in the lid and irritate the surrounding tissues, or it is also related to the vigorous secretion of the blepharophalangeal glands, which can be found in a rounded subcutaneous lump on the eyelid, and a larger lump can make the skin bulge.
3. Sebaceous gland carcinoma of eyelid: when it originates from the blepharoplasty gland, at first it is a small nodule appearing under the skin of eyelid, which needs to be identified because it is similar to blepharoplasty cyst, and after gradual increase in size, it can be diffuse patchy thickening of eyelid, and there is a yellow bulge in the lid conjunctiva.
